Mastering PPH Management (CBT & OSCE Essential)

Whether you are answering a CBT question on the 'Code' or standing in front of an OSCE examiner, knowing your PPH management (The 4 Ts) is non-negotiable.
The 4 Ts of PPH
1. Tone: Uterine atony (The most common cause - 70%). Ask yourself…Is the fundus firm?
2. Tissue: Retained products of conception.
3. Trauma: Tears, episiotomy, or uterine rupture.
4. Thrombin: Coagulation disorders.
OSCE Tip: If you are in the PPH station, remember the 'Communication' aspect is just as important as the clinical. Always explain to the woman what is happening while you are 'rubbing the fundus' and calling for help. 📢
CBT Tip: Memorize the blood loss thresholds. For a vaginal birth, PPH is defined as a blood loss of 500ml or more.
