The HARDEST Procedure This Year! 😱 Huge Aural Polyp & Nasty Infection | Episode 692

This was easily the hardest procedure I've undertaken all year.

In Episode 692, a local swimming pool lifeguard comes in with a severe, painful ear infection producing thick yellow and green discharge alongside an impacted plug of dead skin. Working inside the ear was made exceptionally challenging by a massive aural polyp occupying the canal, leaving virtually no room to maneuver.

Using my smallest suction tube and working with extreme care to avoid disturbing the polyp, watch as we navigate this high-stakes clearance before sending him on for urgent ENT referral and prescribed antibiotic/steroid treatment.

We Found Black Fungal Spores in a Young Swimmer's Ear! | Episode 691

In Episode 691, we treat three patients in the clinic:

Introduction & Patient 1 (Son)
First up is a young lad with large, stubborn skin plugs in both ear canals requiring gentle removal.

Patient 2 (Dad)
While we had him in the chair, we checked his dad’s ears as well! He was half-blocked, so we gave him a thorough cleanout for immediate relief.

Patient 3 (Main Case: Competitive Swimmer with Fungal Infection)
Our main procedure involves a young competitive swimmer who came in with a painful, uncomfortable ear. Upon examination, we discover a thick, blackened skin plug covered with fungal spores (otomycosis)—prompting the question: is it mould? Watch as we carefully clear the obstruction and ensure he gets the right antifungal treatment underway.

His Ear Pain Hid Exposed Bone! (Plus Pharmacist Rescue) | Episode 690

In Episode 690, we examine two very different ear cases:

• Case 1: A gentleman presented with noticeable hearing loss and significant pain. Upon gently clearing away softened keratin buildup, a small crater and area of exposed canal bone were revealed—likely caused by persistent friction and erosion from the keratin. A prompt ENT referral was made for specialist management.

• Case 2: Following an unsuccessful attempt at a pharmacy to clear mushy wax buildup, we performed gentle endoscopic irrigation. Watch the satisfying process as the water gradually breaks down the stubborn wax to reveal a clean, healthy eardrum underneath.

He Ignored an Ear Infection for 6 Months… | Episode 689
In Episode 689, we treat two interesting cases, leading up to a dramatic main procedure dealing with the aftermath of an untreated ear infection.

Case 1: A single-ear wax clearance made challenging by a significantly stenosed (very narrow) ear canal, requiring delicate instrument control.

Case 2 (Main Procedure): Our patient had an ear infection six months ago but never visited a doctor or sought treatment. Although the infection eventually subsided on its own, it left behind an extensive buildup of dried pus and desquamated dead skin adhering to the canal walls. Watch as we perform satisfying peels to clear the canal, followed by gentle endoscopic water irrigation to safely wash away the final debris near the eardrum where suction was too risky.

What Peroxide Drops Actually Do to Impacted Earwax! | Episode 688
In Episode 688, we have a fascinating case of one patient with two completely different ears!

In the first ear, the patient used peroxide drops, which broke down the blockage into a liquid mush—turning it into total "ear soup" that required thorough irrigation to wash clean. In the second ear, no drops were used at all, leaving a solid, tacky impaction that took careful precision using suction, hooks, and curettes to fully clear.

Along the way, we also take a detour into my lifelong love for rugby, from my own playing days to taking the kids to support our local club every week (plus a few rugby jokes thrown in for good measure!).

The DANGEROUS Bedtime Habit That Blocked His Ear! 😱 | Episode 687

In Episode 687 The Wax Wizard, Nurse Richard tackles two fascinating and very different ear wax removals!

First, we see a young girl who unfortunately lost her hearing immediately after using a cotton bud. Thankfully, it's a straightforward extraction and relief is instant!

Next is a much more complex and stubborn case: a massive, tightly packed ball of wax intertwined with dead skin, loose ear trimming hairs, and trapped cotton wool fibres. The patient had been putting cotton balls in his ears every night to block out noise while sleeping—leading to a dense, tangled obstruction deep inside the canal. Watch as Nurse Richard carefully breaks down and extracts this challenging buildup!

The Acid Spray That Dried Out Her Ear! (Plus Tricky Uphill Canal) | Episode 686

In Episode 686, we tackle two fascinating and challenging cases in the clinic, followed by a storytime update from my chaotic weekend trip to Belfast! Patient 1: A long-time patient who has suffered from chronic ear infections her entire life. To keep them under control, she relies on regular acetic acid spray. While it successfully manages the infection, it leaves the ear canal completely desiccated—requiring a delicate peel of large sheets of bone-dry skin every 6 months to clear the canal. Patient 2: A tricky extraction involving a uniquely shaped, uphill-facing ear canal that requires extra precision and care during microsuction and clearance. Weekend Update: Stick around to hear about our weekend getaway to Belfast! Between having a wonderful time and sightseeing, our return flights were cancelled, leading to pure chaos at the airport as we scrambled to get back in time for clinic.
