【膝關節退化治療新趨勢系列（四）】醫語怡情｜物理治療從術前介入　助膝關節置換患者加快康復

當膝關節退化引起的疼痛和活動受限，令步行、上落樓梯，甚至日常起居也變得困難時，病人或需考慮接受手術。然而，除了擔心手術風險之外，不少病人更關心術後何時能恢復活動能力及重返日常生活。

隨着快速康復模式（Enhanced Recovery After Surgery, ERAS）理念在膝關節置換手術中普及應用，物理治療師在病人康復過程中的角色亦愈趨重要。手術前，物理治療師會評估病人的身體狀況、活動能力及生活需要，從而制訂個人化的術前運動計劃，讓他們以更佳狀態迎接手術，同時為術後復康作好準備。

膝關節退化患者大多因疼痛困擾而減少活動，所以容易出現肌力下降及關節活動幅度受限的情況。因此，術前訓練主要集中於強化下肢肌肉及提升關節靈活度，例如鍛鍊大腿四頭肌，以及進行後大腿和小腿伸展運動，有助患者在術後更快自行站立及步行。為加強患者對術前運動的理解，復康中心會提供相關教學影片，方便患者自行於家中進行練習。物理治療師亦會向病人講解術後復康流程，包括助行器的使用方法、安全下床技巧等，當病人更全面了解整個治療過程，便可減輕焦慮和不安，提升康復的信心。

在麻醉及止痛管理技術進步下，病人術後的不適和疼痛已能得到有效控制，有助及早開展復康訓練。一般而言，病人完成手術並待身體狀況穩定後，物理治療師便可於手術當日介入，協助進行膝關節活動及簡單肌力訓練，並指導病人安全下床及正確的起身和步行技巧，有助減少膝關節負擔及活動時的不適。當病人掌握正確技巧後，往往更有信心投入訓練，加快整體復原進度。

手術翌日，患者通常已可在物理治療師的指導下進行站立訓練及練習上落樓梯。及早活動有助促進血液循環、恢復肌力及提升關節活動能力，並縮短住院時間，部分康復進度理想的患者，最快更可於手術翌日出院。

手術後首六至十二星期為關鍵復康期，出院後持續進行運動及練習同樣重要，會影響整體復原進度及成效。患者出院後可按照復康中心提供的運動教學影片在家中練習，物理治療師亦會透過電話跟進患者的恢復進度，了解其有否出現疼痛、腫脹或其他情況，並按需要提供適切建議，協助患者逐步重返日常活動。

不少病人因擔心手術後難以重拾活動能力，因此對接受手術有所猶豫。事實上，臨床上不乏成功個案，而病人積極投入復康訓練亦是康復成效的重要因素。曾有一名熱愛足球的中年病人接受了雙側膝關節置換手術，以重返球場為復康目標。經過逾半年的針對性復康訓練，他的運動能力最終恢復如常，再次享受踢足球的樂趣。

除了手術成效，完善的復康計劃包括術前準備和術後復康訓練，以及病人的積極參與，均有助病人更快康復，重拾自己喜愛的生活方式。

【膝關節退化治療新趨勢系列（三）】醫語怡情｜膝關節置換：從術前到術後管理的麻醉角色

提起麻醉科，不少人可能只會聯想到在手術室讓病人「入睡」的醫生，但其實他們的工作遠不止於此。以膝關節置換手術為例，隨著近年機械臂輔助技術的發展，以及快速康復模式（ERAS）逐漸普及，麻醉科醫生的角色已由手術期間的麻醉管理，延伸至術前評估及術後疼痛管理，貫穿整個治療過程。

接受膝關節置換手術的病人，一般會進行術前麻醉門診評估。麻醉科醫生會詳細了解病人的病歷、長期服藥及過敏紀錄等，並評估心臟、肺部、腎臟等主要器官功能，以識別潛在風險。部分病人即使日常健康狀況良好，仍可能在檢查中發現腎功能下降、較嚴重貧血、血糖或血壓控制未如理想等情況，這未必代表病人不適合接受手術，但可能需先處理或調整藥物，以確保病人安全及避免手術延期。同時，醫生亦會與病人講解合適的麻醉及止痛方案，減輕不確定性帶來的焦慮，有助加強醫患互信。

在ERAS理念下，麻醉管理更強調個人化劑量控制及麻醉深度監察，希望在安全前提下，盡量減少不必要的藥物副作用。現時常採用區域麻醉技術、多模式止痛策略，以及使用較精準的麻醉藥物，以減少噁心、嘔吐、頭暈或過度疲倦等不適，讓病人術後更早恢復飲食和參與復康訓練。

不少病人在考慮是否接受膝關節置換手術時，都會因擔心術後疼痛而感到卻步。現時普遍採用雞尾酒式多模式止痛的方法，即是透過結合不同機制的止痛方式，例如局部麻醉藥、消炎止痛藥、神經阻滯技術等，按每位病人的情況作出組合安排。此方法有助減少單一藥物的劑量，從而降低出現副作用的風險。當疼痛得到有效控制，病人一般更能配合物理治療及早期活動，可加快整體康復進度。

完成手術後，麻醉科醫生仍需觀察病人，以評估其疼痛控制及麻醉消退情況，並留意有否出現噁心、嘔吐、頭暈、喉嚨不適或排尿困難等問題。如病人使用較強效的止痛藥物，醫護團隊亦會密切監察其有否出現嗜睡、便秘、呼吸變慢等副作用，並按需要調整治療方案。術後觀察對於年長或患有多種慢性疾病的人士尤為重要，有助及早發現身體狀況變化，並適時轉介至相關專科跟進。

時至今日，膝關節置換手術已不再是由單一專科醫生完成，而是由骨科醫生、麻醉科醫生、護士、物理治療師等專職醫療人員組成的跨專科團隊，為病人制訂個人化的治療方案。從術前評估、手術安排、疼痛控制、早期活動以至出院規劃，各個環節均需要緊密配合。病人亦應按醫護團隊建議作好術前準備，遵從禁食安排和用藥指示，並主動與醫護人員溝通。當病人、家屬及醫療團隊能夠充分溝通及合作，將有助提升治療成效，讓病人更安心地邁向康復之路。
