【氣胸患者、高齡長者: 是否真的不適合乘搭飛機？✈️Pneumothorax Patients & Elderly Travellers: Are They Really Unfit to Fly?】
很多人以為，只要有機票就能飛，但對於剛經歷過「爆肺」(氣胸) ，的病人，或本身患有慢性阻塞性肺病 (COPD)、哮喘、長期缺氧，或者80、90 多歲的高齡長者👴🏻👵🏻來說，飛行風險遠比想像中高。
飛機升空後🛬，機艙氣壓及氧氣濃度會改變。一般人可能只感到輕微不適，但對肺功能較弱、近期曾氣胸或需要氧氣支援的病人，卻可能引發呼吸困難、血氧下降，甚至誘發哮喘或其他急性狀況。值得注意的是，氣胸有有潛在的復發風險。在高空低壓環境下，一旦再度發生，後果將不堪設想。換句話說，並非「能走能動」就能上機，而是需要經過專業評估、醫療安排及周詳準備。
👩⚕️【真實案例】一位90多歲、患有COPD與哮喘、且剛經歷氣胸的伯伯，需要從香港飛回美國。家人非常擔心，既希望爸爸可以順利回到美國，也害怕長達 13 小時的航程會對他的身體造成風險。
收到個案後，我們第一時間並不是急於安排機票，而是先從醫療安全出發，逐步評估及處理。團隊為病人安排呼吸科專科評估，檢視肺部狀況、用藥需要、氧氣需求及飛行風險；在合適情況下準備「適航證明 (Fit to Fly Certificate)」，並協助向航空公司申請「乘客健康證明書 (MEDA) 」。同時，我們亦安排飛行途中所需的氧氣支援，包括機上氧氣、便攜式氧氣設備、所需藥物、輪椅協助，以及針對長途飛行中濕度變化、血氧監測及哮喘發作風險的預備方案。考量到氣胸可能復發，為應對長途飛行中最壞的情況，我們的醫療團隊特別配置了胸腔穿刺引流儀器，確保能在三萬尺高空提供即時且專業的急救支援。此外，我們也悉心照顧同行的 80多歲太太，讓家人可以真正安心。

💡 對高風險病人而言，飛行不是單純的交通安排，而是一項需要專業判斷、醫療準備及全程守護的任務。
Many people assume that if you have a ticket, you can fly. But for patients who have recently experienced a collapsed lung (pneumothorax), or those living with Chronic Obstructive Pulmonary Disease (COPD), asthma, chronic low oxygen levels — or elderly individuals in their 80s and 90s👴🏻👵🏻 — the risks of air travel can be far greater than most people realise.
Once a plane takes off 🛬, cabin pressure and oxygen concentration change. For most passengers, this causes only mild discomfort. But for those with weakened lung function, a recent history of pneumothorax, or a need for supplemental oxygen, it can trigger breathing difficulties, a drop in blood oxygen levels, or even an acute asthma attack. Pneumothorax also carries a risk of recurrence, and in the low-pressure environment at altitude, that could be life-threatening. So being able to walk and move does not automatically mean you are safe to fly. Professional assessment, medical preparation, and careful planning are essential.
👩⚕️【A Real Case】A gentleman in his 90s, living with both COPD and asthma and having just recovered from a pneumothorax, needed to fly from Hong Kong back to the United States. His family was deeply worried — they wanted him to return home safely, yet feared that a 13-hour flight could put his health at serious risk.
When we received this case, our first priority was not to arrange the ticket — it was medical safety. Our team proceeded step by step: We arranged a specialist respiratory assessment to evaluate his lung condition, medication needs, oxygen requirements, and overall fitness to fly. Where appropriate, we prepared a Fit to Fly Certificate and assisted with the submitting a Medical Clearance (MEDA) to the airline. We also coordinated all in-flight medical support, including in-flight oxygen, portable oxygen equipment, necessary medications, wheelchair assistance, and contingency plans addressing humidity changes, blood oxygen monitoring, and the risk of an asthma episode during the long-haul journey. Given the potential for pneumothorax recurrence, and to prepare for the worst-case scenario mid-flight, our medical team was specially equipped with thoracentesis and chest drainage instruments, ensuring that immediate, professional emergency intervention could be provided at 30,000 feet if needed. We also gave attentive care to his wife, who is in her 80s and was travelling alongside him — so the entire family could fly with genuine peace of mind.
