Gray hair is one of the most recognizable signs of aging. In some people, however, it appears earlier than expected. Premature graying has been linked to stress, certain medical conditions, nutritional deficiencies and, less commonly, some medications [𝟭].

Why does hair lose its pigment? When, as a result of various internal or external factors, the pool of melanocyte stem cells (McSCs) in the hair follicle becomes depleted, or these cells lose their ability to self-renew and differentiate into functional melanocytes, melanin production decreases. Consequently, new hairs grow with reduced or absent pigment, giving them a gray appearance [𝟭–𝟮].

Plucking a gray hair does not make nearby hairs turn gray [𝟭]. The replacement hair from the same follicle will usually also lack pigment. Repeated plucking, however, can injure the follicle and may eventually lead to thinning or permanent hair loss [𝟯].

Hair shedding patients are already sitting in your waiting room - even when they have attended for an unrelated condition. In a study of 300 women attending a public hospital for conditions other than alopecia, 40% of those without female pattern hair loss (104/263) reported excessive hair shedding on hair-washing days. Among women diagnosed with female pattern hair loss, the prevalence increased to 60% (21/37).

𝗧𝗵𝗲 𝗮𝗶𝗺 𝗼𝗳 𝘁𝗵𝗶𝘀 𝘀𝘁𝘂𝗱𝘆 𝘄𝗮𝘀 to assess the prevalence of self-reported excessive hair shedding among women attending a public hospital for conditions other than alopecia and to compare its prevalence between women with and without female pattern hair loss (FPHL). The study included 300 women who were interviewed by a dermatologist and evaluated for the presence or absence of FPHL. Participants self-assessed their hair shedding on hair-washing and non-hair-washing days using the Sinclair Hair Shedding Scale.

In his latest column for Manila Bulletin, Philip Cu Unjieng takes readers inside the beginning of his own TRICOPAT journey under the care of Dr. Julieta Peralta-Arambulo.

Two sessions in, he writes openly about the consultation, the decision to choose a non-invasive approach and what the treatment feels like in practice.

There is no final reveal yet. It is a first-hand account of a process that has only just begun.

Hair extensions are widely used for cosmetic purposes, but they are not without risk. The main concern is traction alopecia, a form of hair loss caused by chronic mechanical tension on the hair shaft and follicle.

Tight or heavy extensions, particularly when worn continuously or repeatedly, increase the risk of follicular damage [1,2].

Early traction alopecia is usually reversible if the source of tension is removed promptly. However, prolonged traction may lead to permanent follicular damage and scarring hair loss [2,3].

The risk depends on factors such as the application technique, extension weight, duration of wear, and individual hair characteristics. Proper application, regular maintenance, and avoiding excessive tension can reduce, but not eliminate the risk [1,3].

𝗧𝗵𝗲 𝗮𝗶𝗺 𝗼𝗳 𝘁𝗵𝗶𝘀 𝗺𝘂𝗹𝘁𝗶𝗰𝗲𝗻𝘁𝗲𝗿 𝗿𝗲𝘁𝗿𝗼𝘀𝗽𝗲𝗰𝘁𝗶𝘃𝗲 𝘀𝘁𝘂𝗱𝘆 was to describe the epidemiological, clinical and trichoscopic features in a case series of patients with an occipital short anagen (OSA), an underrecognized localized phenotype of short anagen syndrome (SAS).

𝗖𝗼𝗻𝗰𝗹𝘂𝘀𝗶𝗼𝗻: This case series describes a previously underrecognized localized phenotype of short anagen syndrome, limited to the central occipital scalp and predominantly affecting females. The clinical presentation, preserved hair texture, and absence of features suggesting hair fragility help distinguish it from acquired hair breakage. Recognition of this entity is important to avoid misdiagnosis and unnecessary treatments.

Rosemary oil has gained popularity as a natural option for hair loss, mainly following a randomized comparative trial suggesting a similar improvement to 2% topical minoxidil after six months in patients with androgenetic alopecia [1].

However, this finding should be interpreted with caution. The study had important limitations, including a relatively small sample size and comparison with a lower-strength minoxidil formulation, rather than 5% preparation. Moreover, clinical data on rosemary oil in androgenetic alopecia remain limited, and the available evidence is not strong enough to place it on the same level as established hair loss treatments [1,2].

Topical minoxidil and oral finasteride remain among the best-studied evidence-based treatments for androgenetic alopecia, whereas rosemary oil should not be considered an evidence-equivalent substitute. Natural products may be considered by some patients who are reluctant to use conventional therapies, but rosemary oil should be regarded, at most, as a possible adjunctive option for selected individuals rather than a direct replacement for minoxidil [2,3].

It should also be remembered that rosemary essential oil and rosemary-derived ingredients may have irritant or sensitizing potential, particularly in susceptible individuals or depending on concentration and formulation [1,2].

𝗧𝗵𝗲 𝗮𝗶𝗺 𝗼𝗳 𝘁𝗵𝗶𝘀 𝗺𝘂𝗹𝘁𝗶𝗰𝗲𝗻𝘁𝗲𝗿 𝗿𝗲𝘁𝗿𝗼𝘀𝗽𝗲𝗰𝘁𝗶𝘃𝗲 𝘀𝘁𝘂𝗱𝘆 was to describe epidemiological, clinical, trichoscopic, and histopathologic features of a localized non-patch form of LPP, based on clinical data from 74 patients.

𝗖𝗼𝗻𝗰𝗹𝘂𝘀𝗶𝗼𝗻: This study pointed out that this condition may represent an early stage of classic or diffuse LPP. Finally, recognizing this presentation is crucial, as timely anti-inflammatory treatment may halt disease progression and scarring. Trichoscopy-guided biopsy of symptomatic yet non-alopecic areas can confirm diagnosis and guide intervention.

Hair thinning, hair loss, and changes in hair texture are common concerns during the menopausal transition and after menopause. Studies show that up to 50% of women may experience these symptoms. They are primarily attributed to a decline in estrogen levels, as estradiol helps prolong the anagen phase of the hair cycle and supports hair growth.

However, the exact mechanisms are complex and may involve genetic predisposition, changes in the hair growth cycle, and increased sensitivity to androgens.

The aging process also reduces scalp metabolism and blood flow to hair follicles, potentially affecting the availability of vital nutrients. In addition to hormonal changes, other factors such as genetics, stress, diet, and the scalp microbiome condition may influence hair health [1,2].

Hair loss during menopause may manifest as diffuse thinning or widening of the parting. The most common diagnoses in these patients include female pattern hair loss (FPHL), frontal fibrosing alopecia (FFA) and telogen effluvium [1–3].

Menopause-related hair loss should not be considered irreversible by default. A thorough clinical evaluation is essential, as coexisting factors such as iron deficiency, thyroid disorders, nutritional deficiencies, or certain medications may contribute to hair loss and require specific treatment. Evidence-based treatments can often slow disease progression, reduce further hair loss, and improve hair density and quality of life [1–4].
