1. To treat the patient.
2. Ideally to prevent the diabetes and also to prevent its complications.
3. If not possible at least to slow down the process of diabetes and its complications.
4. Comprehensive care of diabetes is the ultimate aim.

Privileged to get the invitation to be 'Arogyam' program organized by Rotary Club of Agartala City and my well interacted talk with the honorable rotarians was on "Diabetes A Social Disease" on Saturday afternoon 13th December.

On Sunday afternoon 14th December was invited by All Tripura Diabetes Forum to talk on "Importance of HbA1c" in the 27th Annual Conference at IMA HOUSE, Agartala.

There are nine Key Clinical Points to remember in Diabetes andcFatty live.

Fatty liver is the common term in the understanding of people. Medically known as MASLD (Metabolic Dysfunction Associated Stetotic Liver Disease).

1. "High prevalence in DM"

MASLD affects ~70% of people with type 2 diabetes (T2D); 37% progress to steatohepatitis (MASH), and up to 15% develop advanced fibrosis.

2. "Fibrosis is the key determinant"

Liver fibrosis, not steatosis, drives adverse hepatic (cirrhosis, HCC) and extrahepatic (CVD, CKD, cancers) outcomes.

3. "Screen systematically with FIB-4"

A relatively less frequent presentation is the ‘sausage toe’ where the toe is swollen and erythematous with non-pitting edema. Often the sausage toe is associated with a wound, but that is not always the case.

This condition is highly suggestive of underlying osteomyelitis (OM).

Blood sugars should be kept under control preferably with insulin. If conservative treatments with antibiotics, offloading, regular dressing of the ulcer point are not satisfactory, then extensive surgical debridement is need of the hours with resection of the osteomyelitis involved bones and removal of necrotic tissue.

Offloading would be helpful to avoid such wound, to cure this wound and to avoid high chances of recurrence.

For any individual a positive family history of Cardiovascular disease (CVD) is an independent risk factor for Atherosclerotic cardiovascular disease (ASCVD).

A positive family history of cardiovascular disease (CVD) is typically defined as:

If a parent, brother or sister had a heart attack or stroke at early age (before the age of 55 in males or 65 in females).

Examples of CVD Events
1. Myocardial infarction (MI): Heart attack.
2. Stroke: Ischemic or hemorrhagic stroke.
3. Peripheral artery disease (PAD): Disease of the blood vessels outside the heart and brain.
4. Cardiac revascularization: Coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI).

Importance of Family History
1. Risk assessment: A positive family history of CVD can help assess an individual's risk of developing CVD.
2. Guiding preventive strategies: Knowing the family history can guide preventive strategies, such as lifestyle modifications, risk factor management, and potentially, pharmacological interventions.

In 2024 March this man had left below knee amputation (BKA) followed by lower 1/3rd of thigh amputation to get rid of infection and to avoid life threatening septicemia. He had severe foot infection and healing was impossible because of no blood circulation due to severe peripheral arterial disease (PAD).

This 54 year man is chronic and heavy smoker and this is the main reason for amputation. Both diabetes and hypertension detected in 2024 March. Now after one year blood sugar well under control, however hypertension control is very critical.

Now in 2025 March, in the other leg (right leg) he is having subungual infection in the right great toe as seen in the picture. With lack of blood circulation due to severe PAD in this right leg healing is getting delayed and outcome of surgery would be very difficult. So he is now advised to get revascularization surgery in the form of angioplasty or open bypass surgery to establish proper blood circulation and then only this right leg can be saved from amputation.

With time long diabetes education counselling he stopped smoking and that could save his health from further damage.
