A 18 day old preterm neonate with emergency LSCS and NICU stay for RD and Sepsis was referred for prolonged oxygen dependency.

Echo suggestive of hyperechoic mass around 5 x5 mm above mitral valve.

Differentials
? Infective endocarditis
? Fungal vegetation? Bacterial vegetation
? Thrombus (if there is history of umbilical canulation here it was not done)
?atrial myxoma

Fungal culture sent and iv antifungals continued. Patient now came for 8 day follow up, mass is similar in size. Patient advised to continue antifungal. Surgeons opinion taken

25–26 Weeks Pregnancy
A pregnant mother presented with sudden, severe abdominal pain.

Diagnosis:
🔴 Ovarian Torsion – a gynecological emergency that can lead to ovarian loss and threaten the pregnancy if treatment is delayed.

Early recognition and prompt referral can make all the difference.

⚕️ Timely Surgical Intervention

An emergency laparotomy was performed.

Unfortunately, the o***y was found to be non-viable, and an ovariectomy was necessary.

Key Message:
✔️ The choice between laparoscopy and laparotomy is individualized, depending on:

Gestational age
Maternal and fetal condition
Surgeon expertise
Intraoperative findings
