“When Pneumonia Doesn’t Behave” – A Second Look Saves the Lungs

First procedure at Fortis Healthcare Yeshwantpur, One of the rarest diagnoses.

A 36-year-old woman’s journey to Pulmonary Alveolar Proteinosis

🏥 Act 1: A Familiar Beginning… or So It Seemed
- A 36-year-old woman presented with:
- Progressive breathlessness
- Persistent cough
-🌡️ One episode of fever

Treated as pneumonia 🦠.
📈 Hypoxia improved with IV antibiotics + steroids
❗ Symptoms persisted
📸 Chest X-ray: ongoing bilateral opacities

> Improvement without resolution is often where the real diagnosis hides.

🔍 Act 2: The Diagnostic Pivot
The clinical–radiological mismatch prompted deeper evaluation:
🧩 HRCT → Crazy paving pattern
🫁 Bronchoscopy findings:
🥛 BAL: Milky return
🧪 TBLB: PAS-positive lipoproteinaceous material

➡ Diagnosis: PAP 🎯

💧 Act 3: Why BAL + TBLB Matters
BAL → Suggestive
TBLB → Confirmatory

👉 Together, they:
- Increase diagnostic certainty
- Exclude mimics
- Guide definitive therapy

> In PAP, appearance raises suspicion—histology seals the diagnosis.

💧 Act 4: Targeted Intervention
• With persistent disease but partial physiological stability,
a stepwise, lung-sparing approach was chosen:

🫁 Lobar lavage – Left Upper Lobe
- Targeted clearance of the most involved region
- Reduced procedural burden compared to whole lung lavage
-Allows staged escalation if required

✨ This marked an inaugural interventional procedure in our unit—made even more meaningful by the rarity of the diagnosis.

🌈 Act 5: The Turnaround & What Lies Ahead
Post-lobar lavage:
📈 Improved oxygenation
💨 Reduced dyspnea
📉 Early radiological improvement

➡ Planned next step:
🫁 Whole Lung Lavage for more definitive clearance, based on clinical response

❗ Partial response can mask underlying pathology
🧩 Imaging + BAL + TBLB = strong diagnostic triad
🫁 PAP management can be stepwise and individualized
