Key points focus on safety, proper technique, and verification to prevent complications like aspiration.
Preparation
Gather equipment including disposable gloves, water-soluble lubricant, a clean Ryle’s tube (typically 12-16 Fr size), pH testing strips, a glass of water with straw, adhesive tape, and stethoscope. Measure insertion depth from nostril to earlobe, then to xiphoid process (about halfway between sternum base and navel), marking the tube accordingly—often 50-70 cm for adults.
Insertion Steps
Position the patient upright or semi-upright with head slightly flexed. Lubricate 4-6 inches of the tube tip, insert gently through the chosen nostril aiming backward/downward toward the throat; encourage swallowing with water sips if conscious to guide it into the esophagus. Advance to the pre-marked depth, checking the mouth to ensure no coiling, then secure temporarily with tape.
Placement Verification
Always confirm position before use: aspirate gastric contents and test pH (≤5.5 indicates stomach), perform auscultation (inject air and listen over epigastrium for whoosh), or use X-ray if needed—never skip this to avoid lung misplacement. Monitor for coughing, distress, or resistance during insertion and withdraw if issues arise.
