Tympanoplasty is considered success when reconstructed tympanic membrane is thin without perforation,well epithelialized, and proplerly angled.
Normal social life better hearing water based activity .normal anatomical functional acvivities.

Modified Radical Mastoidectomy — Principle
The core principle is simple:
a)eradicate disease, create a safe ear, and preserve useful hearing whenever possible.

b)Key surgical principles:
Complete eradication of cholesteatoma / diseased epithelium — no hidden disease should be left behind.
c)Create a safe, self-cleaning mastoid cavity by exteriorising the disease-bearing spaces.
d)Lower the posterior canal wall while preserving the facial ridge at a safe level.
e)Exteriorise the epitympanum, aditus and mastoid into one accessible cavity.
f)Preserve the ossicular chain and tympanic membrane whenever disease permits.
g)Protect vital structures — facial nerve, lateral semicircular canal, tegmen, sigmoid sinus and labyrinth.
h)Maintain or reconstruct hearing where feasible, commonly with ossiculoplasty.
i)Achieve long-term surveillance and self-cleaning of the postoperative cavity.

from six months away from the pool to back in action.
A national-level swimmer like Dikshya deserves nothing less than the freedom to breathe, train, compete, and dream without limits. Her journey through adenoid hypertrophy, severe allergic rhinitis, and persistent nasal obstruction and recurrent ear infections was a difficult pause—but never the end of her story.
After successful endoscopic microdebrider-assisted and appropriate immunoglobulin, watching her return to the water is truly heartwarming.

Assisted Adenoidectomy is Precise less Time Consuming faster Healing less Charing of Nasal nasopharyngeal mucosa and less recurrence of Adenoid . The only difference from Coblation is Bleeding but if done proper way precisely proper orientation of scope and saline Irrigation there is less bleeding compared to Coblation.
Adenoidectomy is done in Children if causing nasal obstruction, snoring mouth breathing ,Glue Ear or recurrent chronic otitis media .

Microscopic Tonsillectomy is Precise Bloodless less Time Consuming less tissue Damage less post op pain and faster healing .
The bipolar cautery should be in low power and use more towards the tonsil rather than the Fossa.
If used in proper manner it is as Equivalent like Coblation and within 4_5mins the surgery will be finished .

Intratympanic dexamethasone is a targeted steroid injection into the middle ear, allowing high drug concentrations to reach the inner ear while minimizing systemic steroid exposure.

Gold-standard indications:
Idiopathic sudden sensorineural hearing loss (SSNHL) – as primary therapy, combined therapy, or salvage therapy.

Technique: Inject through the posteroinferior quadrant under microscopy; keep the treated ear upward for 20–30 minutes and advise the patient to avoid swallowing excessively during that period.

Advantages: Delivers a high inner-ear steroid concentration with fewer systemic adverse effects than oral steroids.
Dosage is day 1 3 7 14 21 and 28 days
Repeat PTA After every second Dose.

Endoscopic Microdebrider Assisted
Adenoidectomy is done for sleep Apnea ,snoring Mouth Breathing and Nasal obstruction .

The only fear surgeons use coblation in place of dbrider is Bleeding.
But coblation causes more charring of nasal mucosa tissue sometimes difficult to identify plane and injury torticollis and Nasopharyngeal stenois.

Compared to Debrider if done proper manner complete adenoidectomy quicker removal mild bleeding that can be controlled by Bipolar or coblation

I remove the Adenoid in 3 phages
Phage 1 Gross removal debulking here nasal obstruction part will improve but not snoring or recurrent otitis media and there is High chance of Regrowth.

Phage 2 removal from perimysium and Periosteum region. The perimysial plane shoud be identified in the oropharyngeal isthmus with debrider angulated. This phage can improve snoring mouth breathing.

Phage 3 complete removal from fossa of rosenmuller that can cure recurrent otitis media adhesive otitis media and ear infections.
