UTI, Hyperammonemia & The Renal Connection 🚨

👩‍⚕️ Clinical vignette
A 50-year-old female presents with dysuria, frequency, and lower abdominal discomfort and altered sensorium. No prior liver disease.
👉 Labs reveal elevated ammonia: 159.7 µmol/L

📊 Key lab insights
• Normal bilirubin & transaminases
• Elevated ALP (208 U/L)
• Low albumin (2.7 g/dL)

🧠 What’s happening?
Certain urease-producing bacteria in UTIs break down urea → ammonia (NH₃) → ammonia enters systemic circulation leading to hyperammonemia

🦠 Common culprits
• Proteus mirabilis
• Klebsiella pneumoniae
• Morganella morganii
• Providencia stuartii
• Other urease-producing Gram-negative bacilli

⚠️ Why is ALP important here?
• May indicate renal tubular injury or sepsis-related cholestasis
• Normal LFTs + high ALP → think non-hepatic cause of hyperammonemia

💡 Take-home message
Not all hyperammonemia is hepatic!
👉 In UTI patients with altered sensorium, always think:
UTI → Urease → Ammonia → Encephalopathy
