💊 Metformin vs Glimepiride

Both are commonly prescribed oral antidiabetic medications for Type 2 Diabetes Mellitus, but they work through different mechanisms and have different clinical roles.

✅ Metformin

First-line therapy for most patients with Type 2 Diabetes.

Improves insulin sensitivity and decreases hepatic glucose production.

Minimal risk of hypoglycemia.

Weight-neutral or may promote slight weight loss.

✅ Glimepiride

A sulfonylurea that stimulates pancreatic insulin secretion.

Effective in lowering blood glucose but carries a higher risk of hypoglycemia.

May cause weight gain.

Often used when additional glycemic control is needed.

📚 Clinical Pearl: Metformin is generally preferred as the initial therapy unless contraindicated, while Glimepiride is commonly added when Metformin alone does not achieve glycemic targets.

💡 Question of the Day: Which drug would you choose as first-line therapy for a newly diagnosed patient with Type 2 Diabetes, and why?
