Abstract
Impacted mandibular third molars are a common cause of pain and discomfort. This case report describes the successful surgical extraction of a distoangular impacted mandibular right third molar in a 35-year-old female. Appropriate clinical examination, radiographic evaluation using an intraoral periapical radiograph (IOPA), and surgical management resulted in an uneventful recovery with a good prognosis.
Introduction
Impaction of mandibular third molars is frequently encountered in oral and maxillofacial surgery. Distoangular impactions may present with pain and require surgical removal to relieve symptoms and prevent future complications.
Case Presentation
A 35-year-old female presented with pain in the lower right posterior tooth region. Clinical examination suggested an impacted mandibular right third molar (tooth 48). An intraoral periapical radiograph (IOPA) confirmed a distoangular impaction.
After obtaining informed consent, the patient underwent surgical extraction under local anesthesia using 2% lignocaine with adrenaline. A standard surgical approach was followed, including flap elevation, bone removal as required, tooth delivery, socket irrigation, and wound closure.
Outcome and Follow-up
The patient was prescribed postoperative medications and given routine postoperative instructions. At the 10-day follow-up visit, the surgical site showed satisfactory healing with no evidence of infection or other complications. The patient's pain had resolved completely, and the prognosis was considered good.
Discussion
Distoangular impacted mandibular third molars can be technically challenging to remove depending on their depth and relationship to adjacent structures. Careful preoperative radiographic assessment and meticulous surgical technique are essential for minimizing complications and achieving favorable outcomes.
Conclusion
Surgical extraction of symptomatic distoangular impacted mandibular third molars is a safe and effective treatment. Early diagnosis, appropriate treatment planning, and proper postoperative follow-up contribute to successful healing and an excellent clinical outcome.
