H1N1 INFLUENZA — UPDATED MEDICAL KNOWLEDGE

🦠 What is H1N1?

H1N1 is a subtype of Influenza A. The strain that caused the 2009 pandemic, A(H1N1)pdm09, has subsequently become an established seasonal influenza strain. Therefore, “swine flu” is an older popular term; H1N1 today is generally discussed as seasonal influenza A(H1N1).

It primarily affects the respiratory tract and spreads efficiently between people.

🔬 How does it spread?

Respiratory particles released during coughing, sneezing, talking and breathing

Close or prolonged contact with an infected person

Touching contaminated hands/surfaces and then touching the nose, mouth or eyes

A person may transmit influenza before obvious symptoms begin and is generally most infectious during the early illness.

Incubation: usually about 1–4 days, commonly around 2 days.

🌡️ Common symptoms

Symptoms can begin suddenly:

Fever or chills

Dry cough

Sore throat

Headache

Muscle/body aches

Marked tiredness or weakness

Runny or blocked nose

Sometimes vomiting or diarrhoea, particularly in children

Important: Fever may be absent in some people, especially older adults.

⚠️ Who is at greater risk of complications?

Extra caution is required for:

Children, particularly very young children

Adults ≥65 years

Pregnant and recently postpartum women

People with asthma/COPD or other chronic lung disease

Heart, kidney, liver or neurological disease

Diabetes and other chronic medical conditions

People with weakened immunity

People with severe obesity

🫁 Possible complications

Influenza can occasionally become serious and may cause:

Viral pneumonia

Secondary bacterial pneumonia

Worsening of asthma/COPD

Respiratory failure

Dehydration

Sepsis

Cardiac or neurological complications in uncommon cases

Hospitalisation or, rarely, death

💊 Treatment — the important update

Most otherwise healthy people with uncomplicated influenza recover with rest, adequate fluids and symptom management.

Antiviral medicines such as oseltamivir may be prescribed when clinically indicated.

They are particularly important for people who:

Are hospitalised

Have severe, complicated or progressive influenza

Are at high risk of complications

Best results occur when antiviral treatment is started as early as possible, ideally within 48 hours of symptom onset. However, treatment can still provide benefit after 48 hours in severe illness, hospitalised patients and high-risk individuals.

Do NOT self-start antibiotics. Antibiotics do not treat influenza viruses and are used only when a bacterial infection is suspected/confirmed.

Children and adolescents with influenza should generally not be given aspirin because of the risk of Reye syndrome.

💉 Prevention — strongest protection

Annual influenza vaccination is the key preventive measure. Seasonal influenza vaccines are updated periodically because circulating influenza viruses change.

Other important measures:

Wash hands regularly

Avoid touching your face with unwashed hands

Cover coughs and sneezes

Improve ventilation in indoor spaces

Avoid close contact with people who are ill

Stay home when acutely unwell

Consider a well-fitting mask in crowded/high-risk settings, especially during respiratory-virus surges

Protect vulnerable family members

🚨 Seek urgent medical attention for:

Adults

Difficulty or shortness of breath

Chest pain/pressure

Bluish/grey lips or face

Confusion, severe dizziness or inability to stay awake

Severe dehydration or very little urine

Seizures

Severe or rapidly worsening illness

Children

Fast/difficult breathing

Blue/grey lips or face

Severe dehydration

Unusual sleepiness or unresponsiveness

Seizures

Symptoms that improve and then suddenly become worse

🧪 Diagnosis

Influenza can sometimes be diagnosed clinically, but rapid molecular tests/NAATs and RT-PCR-based testing are generally more sensitive than rapid antigen tests.

A negative rapid antigen test does not always exclude influenza, particularly when influenza is circulating and symptoms strongly suggest it.

🏠 During recovery

Rest adequately

Maintain hydration

Eat nutritious, easily tolerated food

Monitor fever and breathing

Follow prescribed medicines

Avoid strenuous exercise during the acute febrile phase

Gradually return to normal activity after recovery

⭐ Key message

H1N1 is not simply “a severe form of cold.” Most infections are self-limited, but influenza can cause serious complications in vulnerable people. Early recognition + appropriate antiviral treatment when indicated + vaccination + infection-control measures are the foundation of modern H1N1 prevention and management.
