Things to Look Out for Before Tooth Extraction:-
1. Complete Medical History
• Hypertension, diabetes, heart disease, asthma
• Bleeding disorders (hemophilia, anemia)
• Thyroid disorders, epilepsy
• Pregnancy status
• History of infective endocarditis or prosthetic heart
• valves
2. Drug History
Anticoagulants / antiplatelets (aspirin, clopidogrel,warfarin)
• Bisphosphonates or anti-resorptive therapy
• Steroid therapy (long-term)
• Recent chemotherapy or radiotherapy
• Allergy to local anesthetics, antibiotics, or analgesics
3. Vital Signs
• Blood pressure (postpone elective extraction if uncontrolled)
• Pulse rate
• Blood sugar levels (especially in diabetics)
• Oxygen saturation if medically compromised
4. Dental & Local Examination
• Tooth position and mobility
• Root morphology (curved, dilacerated, multiple roots)
• Proximity to vital structures (sinus, inferior alveolar nerve)
• Presence of acute infection, abscess, or cellulitis
• Mouth opening and gag reflex
5. Radiographic Assessment
• Periapical / OPG evaluation
• Root length, shape, number
• Bone density and pathology
• Relation to adjacent teeth and anatomical landmarks
6. Risk of Bleeding
• History of prolonged bleeding after dental procedures
• INR value if on warfarin
• Platelet count if indicated
• Local hemostatic measures planned
7. Need for Medical Consultation
• Uncontrolled systemic disease
• Recent myocardial infarction or stroke
• Complex medication regimens
• Immunocompromised patients
8. Patient Preparation
• Explain procedure, risks, and post-op instructions
• Obtain written informed consent
• Advise light meal before procedure (unless contraindicated)
• Stress reduction protocol for anxious patients
9. Antibiotic Prophylaxis (if indicated)
• Cardiac conditions at high risk
• Immunocompromised patients
10. Emergency Preparedness
• Emergency drug kit ready
• Oxygen supply checked
