Against all odds a HOPE Story

Critical Care HOPE Team Saves 18-Year-Old Soniya from Insecticide Ingestion at Solanki Hospital

Soniya (name changed), an 18-year-old female patient, arrived at Solanki Hospital, Bharatpur, on 30/5/23, with a suspected case of insecticide ingestion. Thanks to the swift response of the Critical Care HOPE Team, she received immediate medical attention.

Based on the provided history and physical examination, it was determined that Soniya was suffering from Organophosphorus compound poisoning. Our Critical Care HOPE Team quickly initiated gastric lavage with normal saline and started treatment with pralidoxime and atropine as per protocol.

However, despite resuscitation efforts, Soniya's neurological condition deteriorated over the next 8-10 hours. Understanding the gravity of the situation, Critical Care HOPE Team made the decision to intubate her and provide mechanical ventilator support. During this critical time, we provided detailed counselling to the anxious family, ensuring clear communication and support.

For the next 48 hours, Soniya remained on an atropine infusion, and gradually her condition started to improve. With determination and perseverance, she was successfully liberated from ventilator support. After a total of 5 days of hospitalization, including 2 days of ventilator support, Soniya was discharged in stable condition.

A crucial element in Soniya's case was the ongoing communication with her family members, especially during the late evening hours. By addressing their concerns and alleviating their anxiety, they felt confident in continuing the treatment in their hometown itself.

Based on the findings of this case study, the Critical Care Hope team was able to provide the patient with the necessary medical care within the golden hour, which ultimately resulted in the patient's successful recovery. Saving the life of a Young Mother in limited resources setting with a multidisciplinary team & Critical Care Hope ICU services

Critical Care Hope has been instrumental in saving the lives of many critically ill patients. Kudos to Team Critical Care Hope for saving the life of a 22-year-old female patient who was admitted to Solanki Hospital in a critical condition is a sheer testimony to this.

Mrs. Pooja from Agra delivered a normal baby 15 days ago. But she was presented with repeated episodes of seizures and unconsciousness. On evaluation, she was suffering from high blood pressure and raised WBC count. The Critical Care Hope team, under the guidance of Dr. Rakesh Solanki, immediately started treatment, intubated her, and initiated ventilator support.

After careful evaluation, Mrs. Pooja was diagnosed with postpartum sepsis with eclampsia and managed with anti-epileptic medication, anti-hypertensives, and broad-spectrum antibiotics. When neurology opinion was taken, the possibilities of cerebral venous thrombosis and meningitis were raised. CT venogram was done and found to be normal.

Despite the resource-limited setting in the region, the Critical Care Hope team tailored the therapy according to the resources available and monitored the patient for the next 24 hours. Thanks to their correct clinical judgment and timely intervention, Mrs. Pooja regained full consciousness the next day. After five days of hospitalization including two days of ventilator support, she was extubated and discharged from the hospital in stable condition without any neurological deficit.

The success of this case highlighted the importance of a setup like Critical Care Hope which was a great assistance to Solanki Hospital which was facing challenges of limited medical resources.

Further timely and appropriate treatment can mean the difference between life and death. In fact, it is estimated that 35-41% of patients suffer or die during transportation for critical care services. Thus, without shifting the patient, Critical Care Hope team made possible the patient’s treatment.

The patient’s family shared heartfelt thanks to the dedicated healthcare professionals of Critical Care Hope as well as Solanki Hospital which remained committed to providing the highest quality of care despite the challenges of limited resources and manpower.
