Understanding Common Eyelid Diseases: Blepharitis, Ectropion, Entropion & Trichiasis

The eyelids play a vital role in protecting the eyes, spreading tears evenly across the ocular surface, and maintaining clear vision. When the eyelids become inflamed, turn inward or outward, or develop misdirected eyelashes, significant discomfort and vision-threatening complications can occur.
The infographic highlights four common eyelid disorders: Blepharitis, Ectropion, Entropion, and Trichiasis. Although these conditions may appear similar because they often cause redness, irritation, and watering, each has distinct causes, signs, and treatment approaches.
1. Blepharitis
Blepharitis is a chronic inflammation of the eyelid margins, often involving the eyelashes and meibomian glands. It is one of the most common causes of irritated eyes seen in eye care clinics.
Common Signs
• Red, thickened eyelid margins
• Scales or crusts around the eyelashes
• Meibomian gland blockage
• Foamy tears or debris at the lid margin
Symptoms
• Burning sensation
• Itching of the eyelids
• Foreign body sensation
• Excessive tearing
• Morning crusting
• Fluctuating blurry vision
Possible Complications
• Dry eye disease
• Recurrent styes (hordeolum)
• Chalazion formation
• Marginal keratitis
Management
• Warm compresses daily
• Eyelid hygiene and lid scrubs
• Artificial tears
• Antibiotic or anti-inflammatory treatment when indicated
2. Ectropion
Ectropion occurs when the eyelid, usually the lower eyelid, turns outward away from the eye. This prevents proper contact between the eyelid and the eyeball, leading to exposure of the ocular surface.
Common Signs
• Outward-turned lower eyelid
• Visible conjunctiva
• Poor tear drainage
• Eyelid laxity
Symptoms
• Excessive tearing (epiphora)
• Dryness
• Irritation
• Redness
• Sensitivity to wind and dust
Possible Complications
• Exposure keratopathy
• Chronic conjunctivitis
• Corneal ulceration in severe cases
Management
• Lubricating drops and ointments
• Temporary eyelid taping in selected cases
• Surgical eyelid tightening or reconstruc
