She is four months pregnant. Inside her uterus is a sixteen-week baby - and a lump measuring thirty centimetres by thirty.

We almost never operate during a pregnancy. But no medicine touches a lump this size, and the pain was severe. Left alone, the pregnancy, the mother and the baby were all at risk.

So we operated - laparoscopically, around a live pregnancy.

Will I be awake during my hysterectomy?

Almost every woman asks this before her operation, and it is a fair thing to be afraid of.

When a vaginal hysterectomy is done under spinal anaesthesia, the injection goes into the lower back, and the patient is often awake through the surgery - so the experience stays in her mind.

The operations we do now are under general anaesthesia. She is asleep from the moment she enters the theatre, and she gets up and goes straight to her room. The experience is very smooth.

Why I reach for cold scissors in a difficult adhesiolysis.

Two advantages. Finding the true plane is easier with cold scissors. And they warn you - with bleeding - the moment you cut in the wrong place.

You also feel the tissue. That haptic feedback comes before the cut, not after it. Feel, then cut.

A dermoid cyst clings to the o***y in some places and thins it out in others - so it tears easily.

Instead of the harmonic I usually use for enucleation, I went back to a technique I saw at a conference: instilling diluted vasopressin between the ovarian cortex and the cyst to open up the natural plane.

Find the right plane and the o***y stays where it belongs.
