This was the situation of him, after his first ear surgery outside.
He had undergone an ear operation at another centre in 2023. Unfortunately, even after the surgery, he continued to experience repeated ear discharge.
He was treated with different medications, but the problem kept coming back. As the tympanic membrane hole/perforation persisted even after his 1st surgery.
Eventually, he was advised to undergo a second ear surgery.

🔍 We decided to look beyond just the ear.
Instead of immediately planning another ear operation, we performed a thorough Endoscopic Ear, Nose & Throat examination.
And this examination revealed something very important.
The patient had significant nasal obstruction due to a Deviated Nasal Septum along with Bilateral Inferior Turbinate Hypertrophy.
His nose was significantly blocked.
We explained to him that his nasal obstruction could be an important contributing factor to the persistent middle-ear problem and poor ventilation of the ear.
So, instead of immediately proceeding with another ear surgery, we discussed a different approach.

“Let us correct the nose first and then reassess the ear.”
We explained that simply repeating ear surgery without addressing an underlying nasal/Eustachian tube ventilation problem may not give the desired result in some patients.

And then came the unexpected and wonderful part of his recovery.

Over the following few weeks, his ear discharge stopped and the eardrum healed without requiring the planned second ear surgery.

ভাবুন তো—
প্রথমবার কানের অপারেশন করানোর পরেও সমস্যা ঠিক হলো না…
বারবার কান দিয়ে জল/পুঁজ পড়ছে…
বিভিন্ন ওষুধ খেয়েও সমস্যা বারবার ফিরে আসছে…
কানের পর্দার ফুটোও (Tympanic Membrane Perforation) অপারেশনের পরেও রয়ে গেছে…
আর এবার চিকিৎসক আবার দ্বিতীয়বার কানের অপারেশন করার পরামর্শ দিয়েছেন।
এই অবস্থাই হয়েছিল আমাদের এই রোগীর।
তিনি ২০২৩ সালে অন্য একটি চিকিৎসাকেন্দ্রে কানের অপারেশন করিয়েছিলেন। কিন্তু অপারেশনের পরেও তাঁর বারবার কান দিয়ে স্রাব/পুঁজ পড়তে থাকে।
বিভিন্ন ধরনের ওষুধ দেওয়া হলেও সমস্যার স্থায়ী সমাধান হচ্ছিল না।
কানের পর্দার ফুটোও প্রথম অপারেশনের পর পুরোপুরি বন্ধ হয়নি।

🔍 আমরা শুধু কান নয়, পুরো সমস্যাটাকে খুঁজে দেখার সিদ্ধান্ত নিলাম।
সরাসরি দ্বিতীয়বার কানের অপারেশন করার পরিকল্পনা না করে, আমরা রোগীর কান, নাক ও গলা Endoscopic Camera দিয়ে ভালোভাবে পরীক্ষা করি।
আর তখনই একটি গুরুত্বপূর্ণ বিষয় সামনে আসে।
রোগীর নাকে ছিল Deviated Nasal Septum (নাকের হাড়/পর্দা বেঁকে যাওয়া) এবং দুই পাশের Inferior Turbinate-এর অতিরিক্ত বৃদ্ধি, যার কারণে তাঁর নাক দিয়ে শ্বাস নিতে যথেষ্ট সমস্যা হচ্ছিল।
অর্থাৎ, তাঁর নাক যথেষ্ট পরিমাণে বন্ধ ছিল।
আমরা রোগীকে বুঝিয়ে বলি যে, নাকের এই বাধা এবং এর সঙ্গে যুক্ত Eustachian Tube-এর স্বাভাবিক বায়ু চলাচলের সমস্যাও তাঁর দীর্ঘদিনের কানের সমস্যার ক্ষেত্রে একটি গুরুত্বপূর্ণ contributing factor হতে পারে।
তাই আমরা সঙ্গে সঙ্গে দ্বিতীয়বার কানের অপারেশন না করে একটি অন্য পদ্ধতির কথা ভাবলাম।

👨⚕️ আমাদের পরামর্শ ছিল খুব সহজ:
“আগে নাকের সমস্যাটা ঠিক করি। তারপর কান আবার পরীক্ষা করে দেখি।”
আমরা তাঁকে বুঝিয়ে বলেছিলাম যে, কিছু রোগীর ক্ষেত্রে কানের সমস্যার পিছনে থাকা নাক ও Eustachian Tube-এর ventilation-এর সমস্যা ঠিক না করে শুধু আবার কানের অপারেশন করলে প্রত্যাশিত ফল নাও পাওয়া যেতে পারে।
রোগী আমাদের উপর বিশ্বাস রাখলেন।
তিনি করালেন Endoscopic Septoplasty, অর্থাৎ Endoscopic পদ্ধতিতে নাকের বেঁকে থাকা Septum সংশোধন করে নাকের বাধা দূর করার অপারেশন।

অপারেশনের পর কয়েক সপ্তাহের মধ্যেই—
✅ কান দিয়ে স্রাব/পুঁজ পড়া বন্ধ হয়ে গেল।
✅ কানের পর্দার ফুটো নিজে থেকেই ভালো হয়ে গেল।

সঠিকভাবে মূল্যায়ন করে প্রথমে নাকের সমস্যার চিকিৎসা করার সিদ্ধান্ত নেওয়া হয়।
