Small cervical fibroid detected, was not seen in routine tvs.

On routine tvs, there was suspicion of cervical lesion. Gel sonovaginogram done: Well defined mass seen arising from cervix involving lower uterine segment and projecting into va**na.

36 year old female with history of recurrent renal stones on medications.
We did a NCCT KUB, got both kidneys full of stones.
I just look at her spine in bone window, n found it abnormal with bone density appearing on higher side than usual. Got a clue, n did a USG neck. A big cystic lesion was there.
Made a PROVISIONAL diagnosis of Parathyroid adenoma leading to PRIMARY HYPER-PARATHYROIDISM leading to recurrent renal stones and bony sclerosis, I just mention my usg findings in my CT report. Patient got his PTH levels done, came out to be 50 times than normal. Successfully operated in PGI n that came out to be Parathyroid adenoma on Histopathology

Patient is on follow up now, comes to me and everytime she comes, her husband gives a big smile n says doc how u came to know that problem was in neck n everywhere I go, people wonder how u came to know about this hidden disease n than he says wat people in old era used to say" Doctors are next to god".

Learning points
* Always look for spine in bone window in every case of CT abdomen. We can add to diagnosis or sometimes can discover new things
* In patients with recurrent renal calculi, advise patients for PTH levels. 1 out of 10 might be suffering from hyper-parathyroidism, primary or secondary.

Endoscopy report shows: Scope cudnot go beyond 27 cm from incisor.
CT with oral contrast done
Esophageal web reported leading to focal narrowing of esophagus
# Learning points #
Esophageal webs are mucosal folds, quite uncommon, can be partial/ complete.
For better detection, can reformat CT images in MIP.
Gud dilatation by oral contrast is must.

Bilateral MCDK is fatal for life and often associated with syndromes including Meckel-Gruber, Zellwegar, Joubert related disorders.

Learning pts.
Common in lower thoracic/ lumbar region
Most reliable sign=Presence of extra-echogenic focus in midline between fetal spinal posterior elements
Associated with meningocele, meningomyelocele, tethered cord.
20 % CASES are associated with ORTHOPEDIC SYNDROME, in which cord divides unequally with the smaller lower limb on same side as the smaller cord.
