Technique-Focused: Single Radial Access for Successful Antegrade Recanalization of a Right Coronary Artery Chronic Total Occlusion in an Elderly Male: A "Low-Contrast" Approach to Minimize Renal Risk.

Chronic Total Occlusion (CTO) Percutaneous Coronary Intervention (PCI) is a specialized procedure used to revascularize a coronary artery that has been completely blocked (TIMI 0 flow) for at least three months. Historically considered the most technically challenging subset of PCI, contemporary success rates have improved significantly due to advanced equipment and dedicated crossing algorithms.
* Access and Approach: While complex cases often require dual access (bifemoral or biradial) for contralateral injections to visualize the distal vessel, a single radial access can be utilized in selected cases to reduce vascular complications and improve patient comfort.
* Antegrade Technique: The antegrade approach is the most common initial strategy, particularly for lesions with a clear proximal cap. It involves "forward" wiring using specialized microcatheters and a wire escalation (AWE) strategy—starting with soft, polymer-jacketed wires to find microchannels and progressing to stiffer, high-penetration wires if needed.
* Low-Contrast Strategy: In elderly patients or those with renal impairment, a low-contrast technique is critical to prevent Contrast-Induced Nephropathy (CIN).

⚡️ "Bailout Stenting Using Guide Extension Support for Calcified Triple Vessel Disease Following Malignant Arrhythmia During Lesion Preparation." ⚡️
Patient Profile:
👩‍🦳 65F
🏥 CC: Severe Chest Pain
📈 Troponin: High (NSTEMI)
📉 ECG: Anterior Wall Ischemia
🫀 EF: Preserved
Diagnostic Findings:
Coronary Angiography revealed Calcified Triple Vessel Disease (TVD).
The Procedure:
1️⃣ LAD Access: Attempted wire crossing. Faced resistance due to a steep calcified bend in the mid-LAD segment.
2️⃣ Support: Successfully crossed distal LAD using balloon support 🎈.
3️⃣ Lesion Prep:
* Started with NC Balloon → Inadequate expansion ❌
* Escaled to Cutting Balloon 🔪
* Switched back to NC Balloon for final preparation.
The Complication:
⚠️ During the 3rd phase of NC inflation, the patient went into Ventricular Tachycardia (VT) 📉.
🔍 Immediate Check: Urgent left injection to rule out perforation. Result: Slow flow (TIMI I-II) but no leak.
The Bailout:
🚀 Technique: Rapid deployment of a Drug-Eluting Stent (DES) using a Guide Extension catheter for extra support.
✅ Result: Post-dilation achieved TIMI III Flow with excellent final angiographic appearance.
