I didn't set out to write.

I had simply run out of reading material.

A warm bath. A leftover mimosa from a 90th birthday breakfast. A few quiet hours in the Harz. No agenda.

Somewhere between my buzzing eight-year-old and my slowly decelerating 90-year-old grandmother-in-law, I realised that I'm no longer rushing to become someone.

I'm learning to become present.

Perhaps that's what forty is asking of me.

Not to do less.
Not to achieve less.
But to remember that who we are doesn't disappear the moment we stop producing.

PCOS has a new name. And I think this matters.
On 12 May 2026, PCOS became PMOS: Polyendocrine Metabolic Ovarian Syndrome.
Because it was never just about cysts. Or even just about the ovaries.
Hormones. Metabolism. Insulin resistance. Cycles. Skin. Mood. Long-term health.
The bigger picture matters.
And perhaps, finally, the name is beginning to reflect it.
For me, this is also a reminder of why I keep talking about context.
A diagnosis is a starting point.

But they are often not the enemy.
They are a message.

In the lead-up to every birthday since I turned 30, I’ve had this little habit of looking back.

Sometimes, its dramatic “What is the meaning of life?” and other times it’s more like: "Okay, what did we learn this year?"

This time I zoomed back two decades, to just as I was entering adulthood. Here’s what I would tell 20 year old Nicola:

Worry less about what the world will say.
Focus on your truth.
They will get their bit in anyway.
Don’t give up the sports that made you feel you could fly.

Money does not need to be feared.
It just needs to be understood.

In the big scheme of things, it’s the little things that add up:
Kindness.
Consistency.
Simplicity.
Human connection.

And maybe the biggest one:
You are a force to reckon with,
Don’t clip your own wings!!

The infant microbiome is like a garden.
Some babies begin with rich soil. Others need more tending.
Vaginal birth and breastfeeding can give a strong microbial start, but they are not the only factors. Skin-to-skin, responsive feeding, outdoor play, fibre-rich family food, fewer unnecessary antibiotics, homeopathy, and a calmer nervous system all help the gut ecosystem mature.
As a homeopath and lactation professional, I often look at the gut story when children present with recurring infections, eczema, allergies, constipation, asthma, or “never well since antibiotics” patterns.
Its quickly apparent then how and where the child’s system needs support.
