# 🫁 **OPD LEARNING: When “I Can’t Get a Full Breath” Isn’t a Lung Problem**
A patient presented with:
> **“Doctor, saans poori nahi aa rahi… chest mein balgam/kachra jama hai.”**
He repeatedly raised his shoulders and took deep breaths. Every few minutes he tried to clear the supposed mucus by putting his finger in the throat, with temporary relief.
# # # The interesting part 👇
* SpO₂ **98–99%**
* Pulse **73/min**
* Chest **clear**
* No wheeze
* X-ray **normal**
* Other investigations **normal**
* No exertional dyspnea
* Symptoms mainly **evening**, for 2–3 hours
# # # 🧠 The clinical trap
The temptation is to keep searching for a respiratory diagnosis or prescribe mucolytics/bronchodilators.
But the **pattern** suggested:
**Incomplete-breath sensation → repeated deep breathing → increased respiratory awareness → discomfort → anxiety → more attention to breathing → throat/chest clearing → temporary relief → cycle repeats.**
# # # Working diagnosis
**Dysfunctional breathing + globus sensation with anxiety-autonomic amplification.**
The symptom is **real**.
The patient's interpretation of it as “mucus blocking the chest” may not be.
# # # What matters in OPD
**1. Rule out dangerous disease first.**
**2. Don't dismiss the patient as “it's all in your mind.”**
**3. Stop unnecessary respiratory medication/investigation once appropriate pathology is excluded.**
**4. Discourage repeated finger manipulation and “chasing” the perfect deep breath.**
**5. Teach relaxed, normal breathing and reduce hypervigilance.**
In this clinical picture, **Ignatia 30C** was considered as a homeopathic trial, with observation rather than frequent repetition.
# # # 🔑 Clinical Pearl
> **When the patient keeps trying to “complete” a breath despite normal oxygenation and examination, don't automatically prescribe for the lungs. First look at the breathing pattern.**
**Treat the pattern, not just the number of breaths.**
*For doctors: clinical learning discussion, not a substitute for individual assessment.*
