ALLEN'S TEST FOR AVF

OBJECTIVES:
• The Palmar Arch Test (PAT) also known as Allen's Test, is a specialty test performed to evaluate the patency of the arteries in the hand prior to radial or ulnar artery harvesting in artery bypass procedures or before the surgical creation of an upper extremity hemodialysis fistula or graft.
• The Allen test assesses collateral blood flow to the hands, generally in preparation for a procedure that can potentially disrupt blood flow in either the radial or the ulnar artery.
• These procedures include arterial puncture or cannulation and the artery harvest alone or as part of a forearm flap.

STEPS:
• Instruct the patient to clench his or her fist; if the patient is unable to do this, close the person's hand tightly.
• Apply from Pressure to Radial and Ulnar artery at the same time with your thumbs or finger tips. This temporarily stops blood flow to the hand
• Let patient open & close hand several times. Hand will lose its normal color (appear lighter ” balanced”)
• Loosen pressure on Ulnar artery ONLY (Keep pressure on radial artery)
• Evaluate the return of blood flow from the ulnar artery to the blood
• Normal response Hand return to normal color (appears flushed) within < 5 seconds Blood flow returned in less than 5 seconds
• Positive modified Allen test – If the hand flushes within 5-15 seconds it indicates that the ulnar artery has good blood flow; this normal flushing of the hand is considered to be a positive test.
• Negative modified Allen test – If the hand does not flush within 5-15 seconds, it indicates that ulnar circulation is inadequate or nonexistent; in this situation, the radial artery supplying arterial blood to that hand should not be punctured
• While applying occlusive pressure to both arteries, have the patient relax his or her hand, and check whether the palm and fingers have blanched. If this is not the case, you have not completely occluded the arteries with your fingers.
