An Technique should not make sense only when things are easy. It should make even more sense when things are difficult.

This patient presented with osteoporosis, Lysthesis and significant canal stenosis—a combination in which weak bone and altered biomechanics make every reconstructive decision critical.

In such cases, a large-footprint cage offers an important biomechanical advantage: broader endplate coverage and better load distribution across the vertebral body. This is particularly relevant in osteoporotic bone, where focal stress and implant subsidence remain major concerns.

A larger footprint does not replace meticulous decompression, careful endplate preparation, secure fixation or optimisation of bone health—but it complements each of them.

The true value of an technique is not demonstrated only in ideal anatomy. It is demonstrated when both biology and biomechanics are working against us.

One of the most memorable moments beyond the podium was my interaction with Dr Kern Singh—a spine surgeon whose work and contributions to minimally invasive spine surgery I have followed and admired for years.

It was a wonderful surprise to discover that our roots trace back to the same small village in Bihar. Meeting someone I have long looked up to, thousands of miles into our respective professional journeys, yet connected by the same humble hometown, made the interaction especially meaningful.

From a small village in Bihar to sharing ideas at an international spine meeting—the world can sometimes feel remarkably small.
