I have recently noticed a growing number of AI generated ultrasound images being shared on social media without clear disclosure. Many of these images appear highly realistic and can easily give a false impression to new students and young practitioners learning ultrasound.

The attached gall bladder mass ultrasound image was created entirely using an AI prompt. This highlights how convincingly artificial ultrasound images can now be produced within seconds.

While AI has exciting potential in medical education and simulation, ultrasound training must continue to be grounded in authentic patient scans and real clinical experience. Real world ultrasound is operator dependent and often includes artifacts, technical limitations, patient variability, and imperfect imaging conditions that AI generated images may not accurately represent.

As educators and clinicians, we should encourage responsible use of AI by clearly labeling simulated images and helping trainees differentiate between AI generated visuals and genuine clinical findings.

Important Update:
Polycystic O***y Syndrome (PCOS) has now been officially renamed as Polyendocrine Metabolic Ovarian Syndrome (PMOS) following an international global consensus published in The Lancet. The new name better reflects the endocrine and metabolic nature of the disorder rather than focusing only on ovarian cysts. Students are advised to use and recognize both terms during the transition period in academic and clinical discussions. (thelancet.com)

Optimising the ultrasound machine is one of the most important aspects of good sonography.

You may be given the best ultrasound machine in the world, but if you do not know how to optimise it according to the organ, depth, body habitus, or pathology you are examining, much of your scanning skill goes to waste.

A well-optimised machine can dramatically improve lesion detection, border definition, vascular assessment, and overall diagnostic confidence. On the other hand, poor settings can make even significant pathology look subtle or completely invisible.

Understanding gain, depth, focus, frequency, dynamic range, Doppler settings, harmonics, and probe selection is not a luxury,it is a core skill of every good sonologist.

Often, the difference between an average scan and an excellent scan is not the machine itself… but the person operating it.

After 25 years of practicing and teaching ultrasound, one thing that has become very clear to me is that the learning never ends.

What once felt like a vast subject continues to expand with time. Every passing day of scanning teaches something new, and almost every other day we come across newer entities that were never part of our earlier learning.

Teaching this subject has made this even more evident. When you try to explain concepts in simple terms and guide students through practical situations, you realize how deep and ever-changing this field truly is.

It pushes you to keep updating yourself, to stay honest about what you know and what you don’t, with humility, and above all, respect for your colleagues.

Even today, it feels like I have only touched a few drops in an endless ocean, and I believe most of my colleagues feel the same.

One important aspect of ultrasound reporting that we often overlook is clarity of communication.

Clinicians usually appreciate reports that are detailed, precise, and clinically meaningful. A report should simplify decision-making for the treating doctor, not create additional confusion. Open-ended impressions, excessive ultrasound terminology in the final conclusion, and overuse of phrases like “please correlate clinically” can sometimes reduce the practical usefulness of the report.

We must remember that not every clinician is deeply familiar with ultrasound language. A surgeon, physician, gynecologist, or general practitioner often prefers a simple and direct impression that clearly conveys the likely diagnosis and its significance.

This does not mean oversimplification or ignoring differentials. It means translating imaging findings into clinically understandable communication.

A good ultrasound report should not merely describe images. It should help in patient management.
