CASE FOR REVIEW- A 39/F came to us with thyroid swelling...FNAC had already been done at two places - private and government labs...two different reports- one said Medullary Carcinoma and other said Papillary Carcinoma.. Patient was very distressed....We needed a PAP stain which is of utmost importance to highlight the nuclear features in Thyroid Cancer....So we did a repeat FNAC on request of the patient and performed all stains- Pap,H&E and MGG... Beautiful papillary fragments with cells showing nuclear grooves and intranuclear cytoplasmic inclusions seen. Marked oncocytic metaplasia was there which was giving the look of medullary carcinoma.
We gave the diagnosis of Papillary Thyroid Carcinoma.
Reassured the patient and thanked them for trusting us for the final opinion!!!
