FOOT DROP

Foot drop is a condition in which a person has difficulty lifting the front part of the foot upward during walking. It usually occurs because of weakness or paralysis of the muscles responsible for ankle dorsiflexion and may result from nerve injury, neurological disorders, muscle weakness, or problems affecting the brain or spinal cord.

Causes

Foot drop can develop from peroneal (fibular) nerve injury, particularly around the fibular head, or from sciatic nerve injury. Other causes include lumbar disc herniation, spinal stenosis, stroke, multiple sclerosis, motor neuron disease, diabetes-related peripheral neuropathy, muscular dystrophy, peripheral nerve disorders, trauma or fractures of the lower limb, and post-surgical nerve injury. Identifying the underlying cause is important because treatment and recovery depend on the location and severity of the neurological or muscular problem.

Symptoms

The most characteristic symptom is difficulty lifting the front of the foot while walking. Instructions to Patients

Patients should use the prescribed AFO consistently, particularly during walking and outdoor activities, and wear properly fitting footwear compatible with the orthosis. Perform the prescribed exercises regularly and avoid suddenly increasing activity beyond your current ability. Keep walking areas at home free from rugs, loose wires, clutter, and other tripping hazards. Take extra care on stairs, uneven surfaces, slopes, and crowded environments. Check the skin regularly for redness, pressure marks, blisters, or irritation, particularly when using an AFO. What is Flat Foot?
Flat foot (Pes Planus) is a condition in which the normal arch of the foot becomes reduced or collapses, causing more of the sole to come into contact with the ground. It may be present from childhood or develop later in life. Some people have flat feet without symptoms, while others may experience pain, fatigue, reduced balance, or difficulty with prolonged standing and walking.

Causes
Flat feet may occur because of genetics or family history, excessive foot pronation, weakness of the foot and lower-limb muscles, ligament laxity, obesity or increased body weight, injury or trauma, arthritis or inflammatory conditions, ageing, prolonged standing, or inappropriate footwear. Acquired flat foot may also develop when the muscles or tendons supporting the medial arch become weak or dysfunctional.

Symptoms
Common symptoms include aching or pain around the arch, heel, ankle, or sole of the foot; swelling around the ankle or inside of the foot; foot fatigue or a feeling of heaviness; reduced balance or stability; difficulty standing or walking for long periods; and sometimes pain extending to the knees, hips, or lower back because of altered lower-limb mechanics.

Possible Complications
When symptomatic flat foot is not appropriately managed, it may contribute to plantar fasciitis, Achilles or other tendon problems, heel pain, abnormal stresses on the ankle and knee, hip or lower-back discomfort, reduced walking efficiency, balance problems, and reduced mobility or quality of life. Not every person with flat feet develops complications.

Management depends on the individual’s symptoms, age, foot structure, activity level, and underlying cause. Choose comfortable, supportive footwear with adequate arch support, cushioning, and a stable heel. Maintain a healthy body weight to reduce excessive mechanical stress on the feet. Perform regular calf, plantar-fascia, foot, and lower-limb strengthening exercises. Avoid prolonged standing or walking when symptoms are aggravated and take regular movement breaks. Gradually increase physical activity rather than suddenly increasing walking or exercise volume. Avoid prolonged barefoot walking on hard surfaces if it increases symptoms, and use appropriate support when needed.

Remember:
Flat feet are not necessarily a problem. Treatment is primarily guided by pain, function, progression, and individual needs—not simply by the appearance of the foot.
