Knee Pain
Knee pain is a common musculoskeletal problem that can affect people of all ages. It may develop suddenly following an injury or gradually because of repetitive loading, degenerative changes, muscle weakness, or altered movement patterns. Knee pain can involve the bones, articular cartilage, menisci, ligaments, tendons, muscles, or surrounding soft tissues. Appropriate assessment is important because the treatment approach depends on the underlying cause.
Causes
Common causes of knee pain include osteoarthritis, sports injuries, ligament sprains, meniscal injuries, patellofemoral pain, tendinopathies, overuse, repetitive running or jumping, and direct trauma. Excess body weight can increase mechanical loading on the knee and contribute to symptoms. Muscle weakness or imbalance involving the quadriceps, hamstrings, hip abductors, and gluteal muscles may affect lower-limb biomechanics and increase stress on the knee. Poor movement patterns, prolonged squatting or kneeling, inappropriate footwear, and sudden increases in physical activity can also contribute. Other medical conditions, including inflammatory arthritis, gout, infection, and certain systemic disorders, can cause knee pain and require appropriate medical evaluation.
Symptoms
Patients with knee problems may experience pain around or inside the knee, swelling, stiffness, reduced range of motion, difficulty walking, difficulty climbing stairs, and difficulty squatting or kneeling. Some patients may notice clicking, crepitus, catching, or a sensation of the knee giving way. Morning stiffness may occur, particularly with degenerative or inflammatory conditions. Pain may increase with prolonged walking, running, stairs, squatting, or prolonged sitting. Severe injuries may produce immediate swelling, instability, inability to bear weight, or difficulty fully bending or straightening the knee.
Complications and Nerve Involvement
The knee joint itself does not normally compress the spinal cord or spinal nerves. However, knee pain can coexist with or sometimes be confused with neurological symptoms originating from the lumbar spine. Conditions such as lumbar disc herniation, spinal stenosis, spondylolisthesis, or other lumbar disorders can irritate or compress a spinal nerve and produce referred or radiating pain, numbness, tingling, or weakness extending from the back or buttock through the thigh and sometimes toward the knee and lower leg. Therefore, a comprehensive assessment should distinguish local knee pathology from lumbar nerve-root involvement.
Spinal Cord Compression
Spinal cord compression is not a direct complication of ordinary knee pathology. Serious spinal cord or cauda equina compression may occur because of conditions affecting the spine, such as severe spinal stenosis, large disc herniation, tumor, infection, or trauma. Depending on the level and severity, spinal cord involvement may cause limb weakness, altered sensation, coordination problems, difficulty walking, balance disturbances, or bowel and bladder dysfunction. New neurological deficits or bowel/bladder changes require urgent medical assessment rather than routine knee rehabilitation.
Precautions
Patients with knee pain should avoid suddenly increasing exercise intensity, repetitive high-impact activities, and movements that consistently produce significant pain or swelling. Deep squatting, prolonged kneeling, jumping, or running may need temporary modification depending on the condition. Maintaining a healthy body weight, using appropriate footwear, warming up before exercise, progressing activity gradually, and allowing adequate recovery can reduce excessive joint loading. However, complete inactivity is generally not desirable; appropriately graded movement and strengthening are important for maintaining knee function.
Management
Management depends on the specific diagnosis and severity of the condition. Conservative management may include activity modification, therapeutic exercise, weight management when appropriate, education, and gradual return to normal activities. Ice may help with short-term symptom relief after an acute injury or when swelling is present, while heat may be comfortable for some people with stiffness. Medications such as analgesics or anti-inflammatory drugs may be considered under appropriate medical guidance. Braces, taping, walking aids, or other assistive devices may occasionally be useful depending on the clinical situation. Persistent symptoms, significant trauma, locking, instability, or suspected structural injury may require further medical investigation.
Physiotherapy Management
Physiotherapy is an important component of knee pain rehabilitation. The physiotherapist assesses pain, swelling, range of motion, muscle strength, joint function, gait, balance, movement patterns, functional limitations, and contributing factors from the hip, ankle, and lumbar spine. Treatment is then individualized according to the diagnosis and functional goals.
Physiotherapy may include progressive strengthening of the quadriceps, hamstrings, gluteal muscles, and calf muscles; flexibility and mobility exercises; neuromuscular and proprioceptive training; balance exercises; gait retraining; functional exercises such as sit-to-stand and step-ups; and gradual return-to-sport training. Manual therapy may be used as an adjunct when clinically appropriate. Patient education regarding load management, movement strategies, exercise progression, and self-management is an important part of rehabilitation.
Importance of Physiotherapy
The primary goal of physiotherapy is not simply to reduce pain but to restore knee strength, mobility, stability, confidence, and functional capacity. Progressive exercise can improve muscle capacity and help the patient tolerate walking, stairs, squatting, work, and sporting activities more effectively. Rehabilitation can also address contributing biomechanical factors and improve movement quality.
For conditions such as knee osteoarthritis, an appropriately designed exercise and education program can substantially improve pain and physical function and may help some patients delay or avoid more invasive interventions. Physiotherapy also teaches patients how to manage their condition independently and how to safely increase physical activity.
When to Seek Medical Attention
Medical assessment should be sought when there is severe pain following significant trauma, inability to bear weight, marked swelling immediately after injury, obvious deformity, a locked knee, persistent instability, fever or significant redness and warmth around the joint, unexplained severe pain, or progressive neurological symptoms. If knee symptoms are accompanied by significant back pain, progressive leg weakness, widespread numbness, difficulty walking, or bowel/bladder dysfunction, spinal or neurological assessment is particularly important.
Key Message
Knee pain is often manageable with appropriate diagnosis, education, progressive exercise, activity modification, and physiotherapy. The most effective rehabilitation approach should address the whole kinetic chain—including the hip, knee, ankle, and, when indicated, the lumbar spine—rather than focusing only on the painful joint. Active patient participation, progressive strengthening, appropriate load management, and long-term self-management are central to achieving better function and quality of life.
