A pre-BAV helps to predict how the will seat-in in relation to the annulus, thus avoiding aggressive post-dilatation.
Asymmetric but excellent final valve position with mild PVL.

Repurposing good-ol' hardware.
Elderly lady with bad distal LM bifurcation + Long segment LAD + LCx/OM bifurcation, EF 20%.
Surgical assessment: too high risk.
Impella is cost-prohibitive (and not readily available). IABP won't cut it. No angioplasty without MCS (too high risk).
RotaFlow ECMO to the rescue. Percutaneous ECMO on L/Femoral. Double bifurcation PCI with stent optimization using final kiss and final POTS without worrying about hemodynamic compromise.

TAVR at COVID times:
72 M with severe AS. Underwent CABG 12 yrs back, all grafts patent. Large annulus. Severe calcification of ascending aorta and infra-renal aorta.
TAVR performed at CARE Nampally today using a large 34 mm valve (Evolut R) percutaneously under conscious sedation. In TAVR world, Evolut 34 mm is called a 'frog' as it jumps in an unexpected manner. This frog did jump once, but we were able to place it perfectly on the second attempt. Demonstrated in the video attached.
