من المعروف ان مرضى العجز الكلوي المزمن يصبح عندهم تركيز الكالسيوم بلدم قليل hypocalcemia وكل مشتقات الكالسيوم وفيتامي D ممنوع اعطائها لمثل هكذا مرضى ف شنو الحل ؟؟
الحل هو اعطاء دواء ال Alfacalcidol والمعروف بلصيدليه ب اسم تجاري One alpha وفقط هذا النوع يعطى شنو السبب؟؟وليش سمي بهذا الاسم التجاري؟
Vit D to be active require the liver and kidney to be activated to the active form (1,25 dihydroxy vit D3) ,
وهذا النوع الفعال هو الذي يزيد امتصاص الكالسيوم بلدم ....
يعني فيتامين D يصير بيه hydroxylation على المواقع الكاربونيه رقم 1(تتم في الكليه باضافه هيدروكسي كروب عن طريق انزيم يدعى One alpha hydroxylase ومن اسم هذا الانزيم اشتق الاسم التجاري للدواء اي بما معنى توجد هيدروكسيل كروب جاهزه في هذا الدواء على موقع ال One alpha carbon لذا هو يكون دواء جاهز ما يحتاج الكليه لتفعيل فيتامين دي خاصه وان الكليه فيها عجز مزمن) وايضا على الموقع الكاربوني الثاني رقم 25 (تتم في الكبد )....
اذا ملخص عمل هذا الدواء:: انه الدواء جاهز لاعطائه للمريض بدون ان يحتاج لانزيمات الكليه وانما فقط يحتاج اضافه هيدروكسيل كروب بلكبد على موقع ذره الكاربون رقم 25 دون ان يتم تحميل الكليه زخم يزيد من عجزها ويتحول الى الهيئه الفعاله لفيتامين دي , 1,25-dihydroxyvitamin D3

clomid ®:
pharmacologecal group:
estrogen receptor modulater (ovulation inducing agent)
mechansims of acion:
clomiphene is apartial agonist at estrogen receptor and is
inhibit the action of stroger estrogen and these lead to
increase in the secreation of gomadotropins and and
estrogen by inhibiting estradiol negative feedback effect on
the gonadotropine
clomiphene is stimulaue ovulation in women with
oligomenorrhea or amenorrhea and ovulatory dysfunction
the common disorder is polycystic o***y syndrome
characterize by gonadotropin dependent ovarian
hyperandrogenism associated with anovulation and
infertility .the disorder is frequently accompaied by adrenal
hyperandrogenism clomiphene probably block the feedback
inhibititory influence of estrogen on hypothalamus causing
asurge of gonodotrin which lead to ovulation
clinical use:
used in the treatment of disorder of ovulation in patient who
wish to became pregnant
single ovulation is induce by single curse of treatment
dosage form:
tab 50 mg
Dose:
corse of treatment 50 mg dialy for 5 day start at the onset
of menstruation (on 2nd day of menstruation preferd) or
any time and if the cycle is cease progesterone will induce
withdrawal bleeding
second course is 100mg dialy for 5 day may be given in the
absence of ovulation
the pt are respond to the frist course or second
side effect:
the most common side effect is hot flush headache
constipation hair loss overian enlargement nausea and
vomiting nervous tension depression fatigue breast
tenderness weight gain urnery frequency and heavy me**es
condraindection
not give to pt with enlargement o***y treatment with
clomiphen more than one year is increase the riske of
ovarian cancer.

Fenistil drops are used for the treatment of allergic reactions such as: nettle rash, upper respiratory allergies (e.g. hay fever and vasomotoric rhinitis), as well as drug and food allergies. Fenistil drops are suitable for the treatment of itching caused by skin diseases connected to various rashes (e.g. varicella), and also for insect bites. They can also be used as an additional treatment for eczema and other non-inflammatory skin diseases of allergic origin.
Fenistil drops can be prescribed to adults or children (1 month and older) and are especially suitable for small babies. Fenistil drops should not be exposed to high temperature - they should be mixed into the feeding bottle right before administration. If the child is spoon-fed Fenistil drops can be administered in this manner as they have a pleasant taste.
Dosage:
Average daily dosage (in three doses spread over the day):
Infants up to 1 year, 10-30 drops;
Infants of 1 to 3 years, 30-45 drops;
Children over 3 years, 45-60 drops;
Adults, 60-120 drops.

Panadol Joint Tablets
Use Panadol Joint for effective relief of joint pain such as that associated with Osteoarthritis.
Panadol Joint is a unique bi-layer caplet incorporating an immediate-release and a sustained release dose of paracetamol. Paracetamol is released in 2 stages, with the first stage being released quickly, for rapid onset of action, and the second stage being released slowly over the day or night, to provide prolonged pain relief.
Directions
Adults (including the elderly) and children aged 12 years and over:
• Oral administration only.
• 2 tablets taken three times a day, every 6-8 hours as required, up to a maximum of 6 tablets in any 24-hour period
• Maximum daily dose: 4000 mg.
• Do not exceed the stated dose. Should not be used with other paracetamol-containing products.
• Can be taken with or without food.
Not recommended for children under 12 years of age.
