WHY WE THE HEALTH CARE WORKERS (HCWs) SHOULD UNFAT
Of all the parasites that affect humanity, I do not know of, nor can I imagine, any more distressing than that of Obesity-WILLIAM BANTING
HERE’S THE QUESTION that has always bothered me: Why are there Healthcare workers (Doctors, Nurses, COs, Lab Scientists, Nutritionists, PHOs etc who are fat?)- I was there too. Accepted as authorities in human physiology HCWs should be true experts on the causes and treatments of obesity. Most HCWs are also very hardworking and self-disciplined. Since nobody wants to be fat, we the HCWs in particular should have both the knowledge and the dedication to stay thin and healthy.
So why are there fat Medics? The standard prescription for weight loss is “Eat Less, Move More.” It sounds perfectly reasonable. But why doesn’t it work? Perhaps people wanting to lose weight are not following this advice. The mind is willing, but the flesh is weak. Yet consider the self-discipline and dedication needed to complete an undergraduate degree, medical school, internship, residency and fellowship. It is hardly conceivable that overweight doctors simply lack the willpower to follow their own advice. This leaves the possibility that the conventional advice is simply wrong. And if it is, then our entire understanding of obesity is fundamentally flawed. Given the current epidemic of obesity, I suspect that such is the most likely scenario.
So, we need to start at the very beginning, with a thorough understanding of the disease that is human obesity. We must start with the single most important question regarding obesity or any disease: “What causes it?” We spend no time considering this crucial question because we think we already know the answer. It seems so obvious: it’s a matter of Calories In versus Calories Out. A calorie is a unit of food energy used by the body for various functions such as breathing, building new muscle and bone, pumping blood and other metabolic tasks. Some food energy is stored as fat. Calories In is the food energy that we eat. Calories Out is the energy expended for all of these various metabolic functions. When the number of calories we take in exceeds the number of calories we burn, weight gain results, we say. Eating too much and exercising too little causes weight gain, we say. Eating too many calories causes weight gain, we say. These “truths” seem so self-evident that we do not question whether they are actually true.
We transform the problem into eating too much (gluttony) and/or exercising too little (sloth). Gluttony and sloth are two of the seven deadly sins. So, we say of the obese that they “brought it on themselves.” They “let themselves go.” It gives us the comforting illusion that we understand ultimate cause of the problem. But really! how will I the obese HCW advise a patient to lose weight whereas I am burdened by the same scourge. I am not saying we the health care workers cannot be sick, No. It seems the interventions we are giving out are not working for our clients and ourselves.
Let ancestral wisdom guide us
William Banting (1796–1878), an English undertaker, rediscovered the fattening properties of the refined carbohydrate. In 1863, he published the pamphlet Letter on Corpulence, Addressed to the Public, which is often considered the world’s first diet book. His story is rather unremarkable. He was not an obese child, nor did he have a family history of obesity. In his mid-thirties, however, he started to gain weight. Not much; perhaps a pound or two per year. By age sixty-two, he stood five foot five and weighed 202 pounds (92 kilograms). Perhaps unremarkable by modern standards, he was considered quite portly at the time.
Distressed, he sought advice on weight loss from his physicians. First, he tried to eat less, but that only left him hungry. Worse, he failed to lose weight. Next, he increased his exercise by rowing along the River Thames, near his home in London. While his physical fitness improved, he developed a “prodigious appetite, which he was compelled to indulge.” Still, he failed to lose weight. Finally, on the advice of his surgeon, Banting tried a new approach. With the idea that sugary and starchy foods were fattening, he strenuously avoided all breads, milk, beer, sweets and potatoes that had previously made up a large portion of his diet. (Today we would call this diet low in refined carbohydrates- Keto diet) William Banting not only lost the weight and kept it off, but he also felt so well that he was compelled to write his famous pamphlet. Weight gain, he believed, resulted from eating too many “fattening carbohydrates.”

ARE CARBOHYDRATES ESSENTIAL?-A SIMPLE SCIENCE-BASED CLARIFICATION

In human nutrition, “essential” means the body cannot make a nutrient on its own and therefore must get it from food. Proteins and fats fall into this category; carbohydrates do not. The body needs glucose, but it can make that glucose from amino acids (proteins) and, to some extent, from glycerol via gluconeogenesis. It does not need dietary sugar and refined starch to survive.

This is why the popular idea of a “balanced diet” often misleads people. What the body truly needs is nutrient-dense food, not a fixed percentage of each macronutrient. A metabolically supportive meal prioritizes quality proteins and healthy fats, including naturally occurring saturated fats, because they provide essential amino acids, essential fatty acids, vitamins, and minerals. Carbohydrates, especially refined ones, mainly provide glucose (sugar) with relatively less nutritional value.
For people living with conditions such as type 2 diabetes, type 1 diabetes, epilepsy, obesity, and some mood disorders, a structured reduction of carbohydrates—what is now termed Therapeutic Carbohydrate Restriction (TCR)—can be a powerful evidence-informed tool, when properly supervised. The scientific logic is straightforward: lowering carbohydrate intake lowers glucose load and insulin demand, allowing the body to rely more on stored glycogen and fatty acids, and to produce ketones as an alternative fuel.

From a biochemistry perspective, fats, proteins, and carbohydrates all converge into acetyl-CoA, which then enters the Krebs cycle in the mitochondria to generate ATP. Energy, therefore, does not “come from carbs” alone. Fats are actually the most energy-dense macronutrient (9 kcal per gram versus 4 kcal per gram for protein and carbohydrate), and both fats and proteins can sustain energy production over long periods. The body is designed to draw on stored glycogen and fat reserves between meals; it is not dependent on constant carbohydrate intake for energy.

Modern nutrition messaging has often emphasized “carbs for energy,” leading many people to believe they must continually eat carbohydrates to function. In reality, much of what people experience after eating—feeling “good” or “satisfied”—is driven by hormones and neurotransmitters such as dopamine, not an instant refill of cellular ATP. When eating patterns are driven by habit and addiction rather than true hunger and metabolic needs, metabolic diseases such as type 2 diabetes, obesity, and fatty liver become more common.

So, are carbohydrates required for human survival? No, they are not essential in the strict biochemical sense. Proteins and fats are. Carbohydrates can be useful tools, especially when chosen from whole, fibrous sources and used strategically, but they are not obligatory for life.

TYPE 2 DIABETES IS FULLY PREVENTABLE AND REVERSIBLE

For years, type 2 diabetes has been framed as a chronic, relentlessly progressive condition – a one way street where blood sugars worsen over time until insulin injections become “inevitable.” This narrative has trapped millions into believing they are serving a life sentence with no possibility of remission. Yet this is simply not true. For the majority of people, type 2 diabetes is strongly driven by diet and lifestyle – and what is driven by lifestyle can be modified by lifestyle.

When we step back and look at the evidence and at real human experience, it becomes clear that type 2 diabetes is, in many cases, reversible. People intuitively sense this: when they lose weight, change how and when they eat, and become more metabolically flexible, their blood sugars improve, their medications are reduced, and many achieve full remission. This is not a miracle; it is physiology responding to new conditions in the body.

Because type 2 diabetes is largely a nutritional and lifestyle disease, only genuine dietary and lifestyle change can address its root causes. Medications can play an important role in symptom control and risk reduction, but they rarely create sustained weight loss or restore normal metabolic function. In fact, some of the most commonly used drugs, such as insulin, are well known to promote weight gain in people with type 2 diabetes, leaving many patients feeling that they are moving in the wrong direction even as their prescriptions increase.

The central lever in reversing type 2 diabetes is meaningful, sustained fat loss – especially from the liver and visceral (abdominal) stores. When excess fat is mobilized and burned, insulin resistance improves, the pancreas is less overworked, and blood sugar control can normalize. Medications may help manage numbers, but they do not “cure” a dietary disease. Expecting drugs alone to reverse type 2 diabetes is like turning up to a bicycle race with a snorkel: the tool simply does not match the problem. The real issue is not only the disease itself, but how we choose to treat it.

The same principles that support reversal also apply to prevention. Obesity and type 2 diabetes are closely linked, and while the relationship is not perfect, increased body weight and central adiposity dramatically raise risk. Maintaining a healthy body composition, eating real unprocessed foods, and respecting our natural metabolic rhythms are powerful first lines of defense. Type 2 diabetes is not an inevitable consequence of modern life. The global surge in this disease is relatively recent, accelerating from the late 1980s with shifts in diet, food processing, and lifestyle patterns. If we look back just one generation, we find eating patterns and activity levels that protected most people from developing this condition.
