Cytomegalovirus (CMV) infection symptoms, diagnosis, and treatment?
CMV is contagious to individuals who have not been infected with it previously, but it is not especially easy to get infected. It is weakly contagious.

How long is cytomegalovirus contagious?
The contagious period when the virus is being shed in body fluids may last for months in an infected individual, and the virus may be shed without symptoms at intermittent periods throughout life.
What is the treatment for cytomegalovirus infection?
There is no cure for CMV, and treatment for CMV infection is not necessary for healthy children and adults. Those at very high risk of developing severe CMV infection may be placed on antiviral medication to help prevent CMV disease. This pretreatment is called prophylaxis. This method has helped reduce the number of CMV infections in these patients.
The antiviral medications against CMV include the following:
•Ganciclovir (Cytovene) is the first antiviral medication approved for the treatment of CMV infection. Ganciclovir, given intravenously, is the drug of choice for the treatment of CMV infection. Side effects include fever, rash, diarrhea, anemia, and low white blood cell and platelet counts.
•Valganciclovir (Valcyte) is an oral medication that is activated to ganciclovir in the body and is widely used to prevent CMV infection (prophylaxis). It is used in selected patients for the treatment of CMV infection and is as effective as intravenous ganciclovir in milder cases.
•Foscarnet (Foscavir) is active against CMV by a different mechanism than ganciclovir and is used to treat infections with CMV that are resistant to ganciclovir. It is a second-line therapy for patients who do not tolerate ganciclovir treatment. Foscarnet is toxic to the kidneys and can cause seizures due to an imbalance of minerals and electrolytes.
•Cidofovir (Vistide) is an alternative therapy for patients who have failed ganciclovir and foscarnet treatment. Its use is limited due to toxicity to the kidneys. It is used mainly for the treatment of CMV infection of the eye (retinitis) in patients with acquired immune deficiency syndrome (AIDS) caused by the human immunodeficiency virus (HIV).
•CMV immune globulin contains antibodies (proteins) that are specific to CMV. It may be prescribed to prevent CMV infection in high-risk lung transplant patients when given in addition to ganciclovir. It is also used with ganciclovir to treat CMV pneumonia.

CMV is a common virus in the same family as herpesvirus, and it can infect anyone. CMV spreads by direct contact with body fluids, such as saliva, blood, urine, semen, vaginal fluids, congenital infection, and breast milk. Thus, breastfeeding, blood transfusions, organ transplants, maternal infection, and s*xual contact are possible modes of transmission.
Most healthy people do not experience any symptoms when infected with CMV, and it does not pose a serious health concern. A majority of adults have antibodies consistent with past infection.
People at risk for serious CMV disease usually have a deficiency of cell-mediated immunity.
Most healthy children and adults who do have symptoms will recover from CMV infection without complications and do not require antiviral treatment.

CMV Symptoms?
Fever is considered a temperature above 100.4 degrees F (38 degrees C). A feverish sensation, however, may occur when the body temperature is above the average normal of 98.6 degrees F. (37 degrees C). Fever is part of the body's own disease-fighting arsenal. Rising body temperatures apparently are capable of killing off many disease-producing organisms. For that reason, low fevers should normally go untreated. Although, if the fever is accompanied by any other troubling symptoms, you may need to see your doctor to be certain.
What are the risk factors for cytomegalovirus infection?
Those at risk for CMV include young children and adults who work closely with them, people who undergo blood transfusions, people who have multiple s*x partners, and previously uninfected people who receive a CMV-infected organ or bone marrow transplant. People at risk for complications from CMV infection include pregnant women and their fetus (congenital cytomegalovirus infection) and those with a weakened immune system, such as people who are HIV-infected, individuals who are recent organ transplant recipients, cancer patients, or those who are taking medications that might suppress their immune system. Cell-mediated immunity is particularly important in preventing CMV reactivation and opportunistic infections.

Antidepressant medications: Selective serotonin reuptake inhibitors (SSRIs) are medications that increase the amount of the neurochemical serotonin in the brain. (Remember that brain serotonin levels often are low in depression.) As their name implies, the SSRIs work by selectively inhibiting (blocking) serotonin reuptake in the brain.
Dual-action antidepressants: The biochemical reality is that all classes of medications that treat depression (MAOIs, SSRIs, TCAs, and atypical antidepressants) have some effect on both norepinephrine and serotonin, as well as on other neurotransmitters. However, the various medications affect the different neurotransmitters in varying degrees.
Atypical antidepressants work in a variety of ways. Thus, atypical antidepressants are not TCAs, SSRIs, or SNRIs, but they can be effective in treating depression for many people nonetheless. More specifically, they increase the level of certain neurochemicals in the brain synapses (between nerves, where nerves communicate with each other).
Monoamine oxidase inhibitors (MAOIs) are the earliest developed antidepressants;
Phototherapy, a particularly effective treatment for seasonal affective disorder, entails the individual with depression being exposed to cool-white florescent light at a strength of 10,000 lux for half an hour every day;
Electroconvulsive therapy (ECT): With the ECT procedure, a brain stimulation therapy, a physician passes an electric current through the brain to produce controlled convulsions (seizures);
Transcranial magnetic stimulation (TMS): Another brain stimulation therapy, that induces a brief magnetic field that can change the electrical flow of the brain that is effective in easing symptoms of depression or anxiety.
Psychotherapies: Many forms of psychotherapy are effective at helping depressed individuals, including some short-term (10-20 weeks) therapies.
