β Posturing in Brain Injury: Decorticate vs Decerebrate

π΅ 1. Decorticate Posturing (Abnormal Flexion)
How it looks:
β’ Arms flexed inward over the chest
β’ Hands clenched
β’ Legs extended and stiff
What it means:
β’ Damage is above the brainstem, usually in the:
β’ Cerebral hemispheres
β’ Internal capsule
Why it happens:
The brain injury interrupts pathways from the cortex, so the flexor muscles overpower extensors β causing the patient to bring arms toward the βcoreβ β βde-COR-ticate = toward the core.β
π΄ 2. Decerebrate Posturing (Abnormal Extension)
How it looks:
β’ Arms extended straight down
β’ Hands turned outward
β’ Legs extended and stiff
β’ Body may arch
What it means:
β’ Damage is below the red nucleus, involving:
β’ Midbrain
β’ Brainstem
β’ Pons
Why it happens:
Lower brainstem injury removes flexor control β extensor muscles dominate β causing rigid extension of arms and legs.
β οΈ Clinical Severity
β’ Decorticate = severe injury
β’ Decerebrate = more severe and worse prognosis because the lesion is deeper in the brainstem.
