1. Anesthesia: Administer local anesthesia to ensure patient comfort.
2. Open a flap for optimal site vision
3. Atraumatic Extraction:
• Gently luxate the teeth using elevators, avoiding pressure on the buccal plate.
• Remove the teeth with forceps while preserving the alveolar bone.

4.Socket Debridement:
• Curette the sockets thoroughly to remove granulation tissue.
• Irrigate with saline to ensure a clean surgical site.

5.Osteotomy Preparation:
• Start with pilot drills following the planned trajectory.
• Sequentially enlarge the osteotomies according to the implant protocol.
• Position osteotomies along the palatal/lingual wall to ensure primary stability.

6.Implant Placement:
• Insert implants into sites 24, 26, and 27 with correct 3D positioning.
• Achieve primary stability with insertion torque >30 Ncm.

7. Buccal Gap Management:
• Pack the buccal gaps with bone graft material (xenograft/allograft).
• Compact graft material gently without overpacking.

Gingival recession is not only a common esthetic concern but also a clinical challenge when it compromises tissue health and increases patient discomfort. Recently, I had the opportunity to treat a case of recession affecting the mandibular central incisors using a free gingival graft (FGG) technique.

1. A graft was harvested from the palatal mucosa, precisely tailored to fit the recipient site.
2. The recipient bed was prepared by exposing connective tissue to optimize vascular integration.
3. The graft was carefully positioned and sutured, ensuring minimal tension and excellent adaptation.
4. Postoperative care was emphasized to ensure optimal healing and patient comfort.

Free gingival grafts remain a gold standard for increasing keratinized tissue, but they often raise important questions about patient comfort and esthetics. In your experience, what are your strategies for balancing functional outcomes with esthetic demands in gingival grafting procedures?
