ရှားရှားပါးပါး
Chilaiditi Syndrome သည် အူလမ်းကြောင်း၏ အစိတ်အပိုင်းတစ်ခု (များသောအားဖြင့် အူမကြီး) သည် အသက်ရှူရန် ကူညီပေးသည့် ကြွက်သား diaphragm နှင့် အသည်း အကြား ချော်သွားသည့် ရှားပါးအခြေအနေတစ်ခုဖြစ်သည်။ ၎င်းသည် ပြင်းထန်သည်ဟု ထင်ရသော်လည်း အချို့လူများသည် မည်သည့်ရောဂါလက္ခဏာမျှ မခံစားရဘဲ ဓာတ်မှန်ရိုက်ခြင်း သို့မဟုတ်
စကင်န်စစ်ဆေးခြင်းတွင် မတော်တဆ တွေ့ရှိခြင်းသာဖြစ်သည်။

ယခု လူနာမှာ Chilaiditi's Syndrome with complication GOO ( အစာအိမ်ထွက်ပေါက် ပိတ်ခြင်းပါ တွဲနေခြင်း )
လူနာမှာ Old age, Malnutrition, Fluid and electrolyte imbalance ကြောင့် သေချာရှင်းပြပြီး
laparotomy ခွဲစိတ်ကုသခဲ့ပါတယ် ၊၊

Multiple small cysts at terminal ileum about 1 feet

So Resection and anastomosis of terminal ileum and biopsy

Chilaiditi's is rare,
Chilaiditi's sign seen in 0.025%–0.28% of people on X-rays,
But the symptomatic chilaiditi's syndrome is even rarer, typically occurring in males (4:1 ratio), especially the elderly (median age 60)

The incidence in autopsy series varies from 1 to 2% (range 0.55 to 13%).
the risk of its malignant transformation is extremely low.

အသက် ၄၃ နှစ် လူနာတစ်ယောက်
ညာဘက် ရင်ဘတ် နှင့် ဘယ်ဘက် ဝမ်းဗိုက်ဘေး ဒဏ်ရာရခဲ့ ( Penetrating chest injury, abdominal injury)
ဆေးရုံငယ် တစ်ခုမှာ ညာဘက် ရင်ဘက်ထဲ ပိုက်ထည့်ပြီး ( Rt ICD Tube with underwater sealed bottle ) စစ်ဆေးရာ အစ ( Foreign body) တခု အသည်းမှာတွေ့ ၊ Gas under diaphragm ကဲ့သို့ တွေ့ရာ မိမိတို့ ဆေးရုံသို့ ဆက်လက်ကုသရန် ပို့ပေးခဲ့။ ထိပြီး ၃ ရက်မြောက်။
ရောက်ရှိချိန် တွင် လူနာမှာမောနေခြင်း မရှိ ၊ အဖျားမရှိ ၊ vital signs stable. Abdomen. soft. No peritonism.
X ray. အပေါ်ပုံ အတိုင်း ။ USG (abd) No free fluid.
Pneumoperitoneum. Gas under diaphragm ကဲ့သို့ X ray မှာတွေ့ရ
Emergency operation or not ဆုံးဖြတ်ရတော့မယ်? ??...............
CT (abdomen) ဆက်ရိုက်ဖြစ်ပါတယ် ။ အောက်က CT ပုံ ပါ အတိုင်းဖြစ် ။
အားလုံး ပြီးစီးသည့် အခါ ဆုံးဖြတ်ချက်ကတော့
Conservative Treatment with Antibiotics and High flow O2, Analgesic, Chest physiotherapy
မခွဲဘဲ စောင့်ကြည့်ကုသမှုပြုလုပ်ခဲ့။ လူနာမှာ တိုးတက်ကောင်းမွန်လာပြီဖြစ်ပါတယ်။

အသက် ၇ နှစ် ကလေးလေး မတော်တဆကြောင့်
အဝတ်ချုပ်အပ် မျိုချမိခြင်း (အပ်က သေးသေး

အပ်(Foreign Body )ကိုမျိုချမိ၍ ဆေးရုံရောက်လာခဲ့ရာ
ဝမ်းဗိုက်မှာ အူပေါက်သည့် လက္ခဏာမတွေ့ရ
ပထမဆုံး ဓါတ်မှန်၌ အပ်မှာ ဝမ်းဗိုက်အပေါ်ပိုင်း
( Epigastrium) ၌ တွေ့ရ၍ အစာအိမ်မှန်ပြောင်း
အရေးပေါ်ကြည့်၍ ထုတ်ရန် ကြိုးစားသော်လဲ
အစာအိမ် ၊ အူသိမ် ဒုတိယပိုင်းထိ မတွေ့ရပါ ။
ထို့ကြောင့် NBM, IV fluid, IV antibiotics and
GI ဆေးများပေး၍ Serial Plain X ray (abd) ရိုက်ပြီး
စောင့်ကြည့်ကုသခဲ့ပါသည်။
အစပိုင်း၌ အပ် ( FB) မှာ အူသေး ( Small intestine)
တလျှောက် ရွေ့နေသော်လဲ အူသေးနှင့် အူမကြီး
ဆက်သောနေရာ ( Ileocaecal valve) ကို မကျော်နိုင်ဘဲ
ရှိနေသည့်အတွက်
ဘာလုပ်ရမလဲ? ??????

မရရင်တော့ Open laparotomy ဗိုက်ခွဲပြီး ထုတ်ဖို့ပါ

ဆရာဝန်ရဲ့ ကံ ၊ ကလေး ရဲ့ ကံ နဲ့ ဝိုင်းဝန်း ကူညီသူ
အားလုံးကြောင့် ဗိုက်ခွဲစရာ မလိုဘဲ အူမကြီး မှန်ပြောင်း
မှတဆင့် ခက်ခက်ခဲခဲ နဲ့ ထုတ်လို့ရသွားပါတယ် ။
အပ် (FB) ကို Forcep နဲ့ ကိုင်ပြီး ဖြည်းဖြည်းထုတ်ရာမှာ
အပ်က ကျန်ခဲ့ပြီး ဝမ်းတွေထဲ မြုပ်နေ၍ C Arm X ray
ဖြင့် ပြန်ရှာရပါသေးတယ် ။

GI Surgery အဖွဲ့ အားလုံး ၊ ခွဲစိတ်ခန်းမကြီးနဲ့ မေ့ဆေး
အဖွဲ့ အားလုံးကို ကျေးဇူးတင်ရပါတယ် ။ ကလေးပါရဂူရဲ့ ဝိုင်းဝန်းကြည့်ရှု့ပေးမှု့အတွက်လဲ ကျေးဇူးပါ ။

မျက်လှည့်ပြတဲ့ ဝမ်းဗိုက်လေး Magic Abdomen

အသက် ၄၆ နှစ် အမျိုးသမီး
ဝမ်းဗိုက် ၂ ကြိမ်ခွဲပြီးနောက် ခွဲထားသောအနာနေရာမှ အရည် ( intestinal juice) များ ထွက်လာလို့ မိမိတို့ဆေးရုံ လွှဲပြီးရောက်လာတာပါ ။
စဖြစ်တာက ဝမ်းလျှောတာ ၃ ရက် ၊ နောက် ဝမ်းဗိုက်နာတာ ၂ ရက် နဲ့ ဝမ်းဗိုက်ဖောင်းလာလို့ ဆေးရုံငယ် တခုမှာ သွားပြပြီး စမ်းသပ်စစ်ဆေးရာ အူပေါက်တဲ့လက္ခဏာ ( Peritonism and GUD positive) ကြောင့် အရေးပေါ်ခွဲစိတ်ခဲ့ပါတယ်။ ဝမ်းဗိုက်အတွင်းမှာ ပြည်အရည်များတွေ့ရပြီး အူပေါက်တဲ့နေရာ မတွေ့ရပါဘူးတဲ့။ ဒါကြောင့် ပြည်အရည်တွေထုတ် သန့်ရှင်းရေးလုပ်ပြီး ပိုက် ( Drainage tube) ထည့်ခဲ့ပါတယ်တဲ့။ ခွဲပြီး ၆ ရက်အကြာမှာ ခွဲထားတဲ့အနာမှ အရည်များထွက်လာလို့ ( bile stained intestinal juice) ဒုတိယ အကြိမ် ခွဲခန်းဝင်ပြီး ခွဲခဲ့ပါတယ်တဲ့။ ဒုတိယ အကြိမ်မှာလဲ ဝမ်းဗိုက်အတွင်း ပြည်အရည်များသာ တွေ့ပြီး အူအပေါက်ရှာမတွေ့လို့ ပထမ အကြိမ်ကဲ့သို့ ပြုလုပ်ခဲ့ရပါတယ်တဲ့။ ဒုတိယအကြိမ်ခွဲ ပြီး ၅ ရက်မြောက်မှာ ခွဲထားတဲ့အနာအောက်ခြေမှ အရည်တွေ ထွက်လာလို့ မိမိတို့ဆေးရုံပို့ပြီးရောက်လာပါတယ် ။
ဒုတိယ အကြိမ်ခွဲပြီး ၇ ရက်မြောက် မိမိတို့ဆေးရုံရောက်ရှိချိန်မှာ
လူနာမှာ အဖျားမရှိ ၊ Vital sign -Stable, Abdomen soft. Already inserted Ryle's tube and Drainage tube. Light yellow colour fluid on laparotomy wound dressing. USG ( abd) urgent - Free fluid in interloop and pelvis.

3rd time relaparotomy or not
၂ ခါခွဲထားပြီး လူနာ
Afebrile
Vital sign - Stable
Abdomen soft
But? intestinal juice from wound
USG (abd) free fluid
K level low
အူတော့ပေါက်ခဲ့ပါလိမ့်မယ် ။ ဒါပေမဲ့ အပေါက်ရှာမတွေ့ခဲ့ ။

စိတ်လောပြီး ချက်ချင်းပြန်မခွဲသေးဘဲ
IV fluid and electrolytes replacement, IV antibiotic, GI cover and Nutritional build up လုပ်ပြီးစောင့်ကြည့်ကုသခဲ့ပါတယ်။
လူနာ တိုးတက်လာလို့ နှာခေါင်းပိုက်ဖြုတ် ၊ D/T ဖြုတ် ပြီး oral clear fluid စလိုက်ပါတယ် ။ 2 - 3 oz /Hr ထိ အဆင်ပြေလို့ နောက်နေ့မှာ liquid diet တိုးရာမှာ
D/T site Lt flank က Faecal stained fluid ထွက်လာလို့ Partial bowel rest ပြန်ထားပြီး

အပေါက်ကို ဘယ်လိုရှာရမလဲ
စက်ဘီး ကျွတ်အပေါက်ရှာသလို ရေထဲနှစ်လို့လဲမရ
ခွဲပြီးသားဆိုတော့ GUD X ray ကလဲ အဆင်မပြေ
ပြန်ဝင်ရင်လဲ ၂ ခါခွဲပြီးသားဆိုတော့ ရှာတွေ့ပါ့မလား
လား ပေါင်းများစွာနဲ့ ပါ

နောက်ဆုံးတော့ ဓါတ်မှန်ဆရာဝန်နှင့် တိုင်ပင်ပြီး
Gastrograffin follow through လေးရိုက်လိုက်ပါတယ်
Barium ဆိုရင် အပေါက်ရှိခဲ့ရင် Leak ရပြီး Peritoinitis ရမှာစိုးလို့ Gastrograffin ကို request လုပ်ခဲ့ပါတယ် ။

ပုံ ပါ အတိုင်းလေးတွေ့ခဲ့တာပါ။

Gastrograffin Leak from Descending Colon and lacalized collection about 27 cc.
EC Fistula guideline အတိုင်း ကုသခဲ့ရာ ယခုတော့ ထမင်းစားနိုင်ပါပြီ။ ထပ်မံ မခွဲစိတ်ရတော့ပေမဲ့ ဘာကြောင့် အူမကြီး ပေါက်လဲ ဆိုတာကိုတော့ ဆက်လက်စုံစမ်းရှာဖွေနေဆဲပါ ။

ဓါတ်မှန်ပါရဂူ ဌာနမှူး အစ်မ Daw Thida Sein နဲ့ အားလုံးကို ကျေးဇူးတင်ရပါတယ်။

Prof တစ်ယောက် ပြောကြားဖူးတာလေး သတိရမိပါတယ်။ " အိမ်အဟောင်းကို ပြင်ရတာ အိမ်အသစ်ဆောက်ရတာထက် ခက်တယ် " တဲ့ ။ စိတ်ရှည်ရမှု့ ၊ ပုံမှန်ထက် စဥ်းစားလုပ်ကိုင်ရပါတယ်။

CT (abdomen) မှာ ကင်ဆာနှင့်တူသော လက္ခဏာ ၊ ပုံမှန်မဟုတ်သော အချိုင့် ( Diverticulum) ၊ နာတာရှည် အူမကြီးယောင်ရမ်းသောရောဂါ ( IBD) မတွေ့ရသလို ၊ ယခင် Gastrograffin follow through မှာ တွေ့ရတဲ့ Fistula tract and leakage လဲ မရှိတော့ပါဘူး ။
Tumour markers are within normal limit. Widal test only salmonella typhi o Ag. 1/160 Positive ပါ ။

Now patient went back to her home sweet home with smile face.

အသက် ၆၈ နှစ် မုန့်ကြိုအိတ်( Pancreas ) ကင်ဆာ ဖြစ်နေသော အမျိုးသားကို ခက်ခက်ခဲခဲ ပင်ပန်းစွာ ခွဲစိတ်မှုပါ ။
ခွဲစိတ်အဖွဲ့ရဲ့ တက်ညီလက်ညီ ကြိုးစားမှု့ကြောင့် အောင်မြင်စွာ ခွဲစိတ်နိုင်ခဲ့ပါတယ်

68 year old Gentleman with Ca Head of Pancreas for which whipple operation ( Triple bypass - pancreaticojejunostomy, choledochojejunostomy and Gastrojejunostomy) was done.
Very difficult operation but we can do it because of team spirit.
Really thanks a lot my surgical team, Anaesthetist team,
OT staffs and who support to us. Immediate post -op condition
is stable. Hope for the best.

Biopsy - Pancreatic ductal carcinoma.
Recheck Ca 19.9 and serum Amylase was reduced to normal limit. Patient was carried on chemotherapy.

ခက်ခဲတဲ့ ပန်ကရိယကင်ဆာ ခွဲစိတ်မှု့

61 yrs old Gentleman with Ca Head of pancreas ( Stage I B )
for which Whipple operation was done. The patient also has
Mild to moderate AR and Abdominal Aortic Aneurysm.
Thanks Radiologist , Cardiac physician, Anaesthetist,OT Staff
and my efficient GI surgical team. Hope for the best.

အစာအိမ်မှန်ပြောင်းမှ တဆင့် အရည်များ ဖောက်ထုတ်
ရန် ဆုံးဖြတ်ဆောင်ရွက်ခဲ့ခြင်း အမှတ်တရ ။
