Here’s the complete case from the some days ago story. A 57yo F, who presented multiple oral ulcers and erosions in whole oral mucosa, along with excruciating pain interfering with regular feeding. One of the main complaints was pain from gingival lesions. The lesions had 2 months of evolution. She was treated previously with antifungal agents and low potency topical corticosteroids with no complete resolution. She was programmed for oral biopsy with a provisional diagnosis of blistering disease (pemphigus-pemphigoid). Days after the first visit she develop skin blistering lesions. HP image correspond to oral lesions, showing clear signs of acantholysis and subepithelial cleft, consistent with . Later on, skin biopsy was taken too by dermatologist with similar features consistent with Pemphigus Vulgaris (not shown). Although always direct immunoflourescence is recommended, not always is easy available as it was in this case. CLINICAL TIP TO REMEMBER, 50% of Oral Pemphigus cases starts-debuts with oral ulcerative lesions before skin blistering lesions appear. Useful keep in mind that oral Pemphigus lesions are the “fist to come and last to go”. Hope you like it! 🤓😉👍
