The Chameleon of the Heart

A young, fit gentleman, in his forties, no medical illness previously, non-smoker, was referred by our friendly neighbourhood doctor, for cardiac assessment after presenting to his clinic with typical symptoms of gastritis- central chest pain relieved completely after burping, associated with sweating after taking spicy food.

Based on his symptoms, he would have been discharged home with gastric medications. But, the vigilant doctor decided to play safe and refer although his ECG was normal.

At the hospital, ECG done was still normal but his cardiac troponin was elevated.

Proceeded with urgent coronary angiogram noted severe single vessel disease involving the widowmaker artery. If he was treated as gastritis, one day his wife may end up becoming a widow if he gets into a massive heart attack.

📝 Moral of the story: Do not assume every gastritis symptom is definitely due to gastritis. Please get yourself checked by a doctor even if it’s simple gastritis.

🧪 Clorox: A lifesaver or disaster?

Recently a 72 year old lady previously very healthy, a vegetarian with no known medical illness, presented to me with sudden onset shortness of breath but no chest pain after accidentally inhaling Clorox while washing her house toilet.

She was diagnosed to have acute heart failure. Her Electrocardiogram and Echocardiogram were normal but her cardiac troponin was slightly elevated.

Hence, an angiogram was performed for her due to raised troponin and noted she has severe two vessel disease. Had she not detected this earlier, she could have suffered from a major heart attack out of the blue.

She was discharged well after a successful angioplasty. 😊

Moral of the story, please always do health check-up once you are in your golden years even if you have no health issues.

A middle aged lady with newly diagnosed hypertension was referred by my colleague for cardiac assessment when she presented initially with giddiness sounding like vertigo and occasional non-specific palpitations.

No shortness of breath, No chest pain, No typical symptoms of angina.

ECG and Echocardiogram were completely normal.

Decided to work her up for coronary artery disease due to her risk factors.

CTCA showed moderate disease and proceeded with angiogram which noted so severe LAD disease.

Subsequent angioplasty was done and she went home feeling great and thankful that early detection of significant coronary artery disease made a difference in her life.

📝Moral of the story: Awareness is the key to early detection of coronary artery disease.
