🌟 KISAH KEJAYAAN RAWATAN SCOLIOSIS

Seorang kanak-kanak lelaki berusia 12 tahun hadir ke klinik kami dengan Cobb's Angle 50° dan rib hump yang ketara.

Selepas 3 bulan menjalani rawatan konservatif:

• Cobb's Angle 50° ➜ 16° (X-ray semasa memakai brace / in-brace)
• ATR Thoracic 25° ➜ 11°
• Postur badan semakin seimbang.
• Rib hump semakin berkurang.

🚨 “Noticing your child’s posture looks uneven?

Don’t ignore it.”

Scoliosis often starts during the teenage years (10–15) — and can progress silently without obvious symptoms.

Early signs:
❌ Uneven shoulders
❌ Uneven waistline
❌ Body leaning to one side
💔 If detected late, your child may require spinal surgery.

💡 Early treatment can help control progression without surgery.

👣 PLANTAR FASCIITIS – COMMON BUT TREATABLE!

Do you feel heel pain when you wake up in the morning or after long standing/walking?
You may have Plantar Fasciitis.
Plantar fasciitis is one of the most common causes of heel pain. It happens when the plantar fascia (thick band of tissue under your foot) becomes inflamed due to:

✔️ Prolonged standing
✔️ Overuse / excessive walking
✔️ Flat feet or high arch
✔️ Tight calf muscles
✔️ Improper footwear

🎯 Treatment Goals:

✅ Reduce pain & inflammation
✅ Improve flexibility (calf & plantar fascia)
✅ Restore normal walking pattern
✅ Improve strength & foot stability
✅ Help you return to daily activities comfortably

💪 Physiotherapy Treatment May Include:
• Manual therapy
• Stretching & strengthening exercises
• Taping technique
• Shockwave therapy
• Ultrasound therapy
• Footwear advice

Early treatment prevents chronic pain!

Upper Crossed Syndrome (UCS) – Silent Cause of Neck & Shoulder Pain

Do you often feel:

✔️ Neck pain or stiffness
✔️ Tight shoulders
✔️ Upper back discomfort
✔️ Poor posture when sitting or using your phone?

These may be signs of Upper Crossed Syndrome (UCS) — a common postural problem caused by muscle imbalance.
🔹 Tight muscles: upper trapezius, levator scapulae, chest muscles
🔹 Weak muscles: deep neck flexors, lower trapezius, serratus anterior

If not treated early, UCS can lead to chronic pain, headaches, shoulder problems, and long-term posture issues.

How Physiotherapy Helps
✅ Detailed posture & movement assessment
✅ Stretching tight muscles
✅ Strengthening weak muscles
✅ Postural correction & ergonomic advice
✅ Exercise-based rehabilitation

✨ Early treatment helps restore posture, reduce pain, and prevent future problems.

🫁 Scoliosis & Lung Function:

What Happens in a Single Thoracic Curve?

Many people think scoliosis only affects posture — but if the curve is in the thoracic spine (mid-back), it can also affect how the lungs expand and function.
When the spine curves and rotates, the rib cage becomes uneven:
🔹 One side of the chest collapses inward
🔹 The other side becomes over-expanded
🔹 The lung on the concave side has less space to expand

📉 Scientific evidence shows:
Research has demonstrated that thoracic scoliosis can lead to a restrictive breathing pattern, where total lung capacity and vital capacity are reduced due to rib cage stiffness and reduced chest wall mobility.
Studies also found that:
✔️ The larger the Cobb angle, the greater the reduction in lung function
✔️ Thoracic curves are more likely to affect breathing compared to lumbar curves
✔️ Early intervention can help preserve chest mobility and respiratory efficiency

💨 This means even without obvious breathing symptoms, lung expansion can already be affected — especially in growing children and teenagers.

✅ The good news:
Curve-specific physiotherapy such as Schroth-based scoliosis rehabilitation focuses on: • Rotational breathing into the collapsed side of the lung
• Improving rib cage mobility
• Enhancing postural alignment
• Supporting better breathing efficiency

📣 Early screening + early intervention matters.

Don’t wait until the curve becomes severe — because protecting lung function is just as important as correcting posture.

In the Schroth Method, breathing is NOT normal deep breathing.

👉 It is a specific corrective breathing technique called
Rotational Angular Breathing (RAB)

🔍 Why is breathing important in scoliosis?
Scoliosis is a 3-dimensional deformity:
✔️ Side bending
✔️ Spinal rotation
✔️ Rib cage asymmetry (one side collapsed)

Schroth breathing is used to:
✅ Expand the collapsed (concave) side of the chest
✅ Reduce spinal rotation
✅ Support 3D postural correction

💡 How is Schroth breathing different?
❌ Not random breathing
❌ Not forceful deep breaths
✔️ Breath is directed
✔️ Inhale into the weak / sunken side
✔️ Exhale while maintaining correction
✔️ Combined with posture & corrective exercises

👨‍👩‍👧 Simple explanation for patients & parents:
“Schroth breathing trains the body to send air into the collapsed side of the ribs to help straighten and de-rotate the spine.”

⚠️ Important to know
Breathing alone does not correct scoliosis.
It must be combined with:
✔️ Schroth corrective exercises
✔️ Postural awareness
✔️ Bracing (when indicated)

✨ Scoliosis needs more than exercise — it needs 3D correction.

🧠 STROKE REHABILITATION: DO IT RIGHT, FROM THE START

A good therapist always starts with stretching.
Why? Because stroke muscles are tight, spastic, and unbalanced.

✅ Stretching first
• Reduce spasticity
• Improve joint range
• Prepare the affected side
• Prevent stiffness & contracture

👉 Then we activate the affected side
• Joint approximation for stability & proprioception
• Weight-bearing to wake up weak muscles
• Proper alignment before movement training

🎾 Bounce ball training
• Improve coordination & reaction
• Enhance hand control
• Train focus on the affected side
• Fun but clinically powerful

📌 Rehab is not random exercise.
📌 Rehab is sequence, skill, and strategy.

💪 Don’t let the strong side compensate.

🧠 Train the brain. Activate the affected side.
