Neurosurgery HSIP Performs Kelantan’s First Awake Craniotomy for High-Grade Glioma in MOH Facility

KUALA KRAI — The Neurosurgery team successfully performed the first awake craniotomy for a high-grade glioma in the Ministry of Health (MOH) Kelantan, marking a significant milestone in neurosurgical services in the state.

The complex procedure was carried out at Hospital Sultan Ismail Petra in Kuala Krai by the dedicated Neurosurgery Unit, led by Mr Ridzuan, Head of Unit Neurosurgery HSIP, together with the Anaesthesia team led by Dr Azhar, Head of the Anaesthesia Department HSIP, with support from Prof Hasyizan, visiting Neuroanaesthesiologist from HPUSM. The procedure was further supported by the operating theatre staff and allied health professionals.

Awake craniotomy is an advanced neurosurgical technique performed while the patient remains conscious during critical stages of the surgery. This allows surgeons to continuously assess important brain functions such as speech and movement while removing the tumour, helping to maximize tumour resection while preserving vital neurological function.

The patient, diagnosed with a High-grade glioma, underwent the procedure successfully and remained neurologically stable throughout the operation. Continuous intraoperative neurological assessment enabled precise tumour removal while safeguarding eloquent brain areas.

This achievement represents a major advancement in neurosurgical capability in Kelantan, reflecting the growing expertise and commitment of the Neurosurgery team to provide cutting-edge care for patients with complex brain tumours.

The success of this procedure highlights the importance of multidisciplinary collaboration and marks another step forward in expanding advanced neurosurgical services within the Ministry of Health.

The Neurosurgery Unit expressed appreciation to all teams involved and reaffirmed its commitment to delivering high-quality neurosurgical care for patients in Kelantan and the surrounding regions.

Unit Neurosurgeri HSIP Melaksanakan Awake Craniotomy Pertama di Kelantan bagi Pesakit High-Grade Glioma di Fasiliti KKM

KUALA KRAI — Pasukan Neurosurgeri telah berjaya melaksanakan pembedahan awake craniotomy pertama bagi kes high-grade glioma di fasiliti Kementerian Kesihatan Malaysia (KKM) di Kelantan, sekali gus menandakan satu pencapaian penting dalam perkhidmatan pembedahan saraf di negeri ini.

Prosedur kompleks ini telah dijalankan di Hospital Sultan Ismail Petra, Kuala Krai oleh Unit Neurosurgeri yang diketuai oleh Mr Ridzuan, Ketua Unit Neurosurgeri HSIP, bersama pasukan Anaestesia yang diketuai oleh Dr Azhar, Ketua Jabatan Anaestesia HSIP, dengan sokongan Prof Hasyizan, Pakar Neuroanaestesia pelawat dari HPUSM. Prosedur ini turut mendapat sokongan daripada kakitangan dewan bedah serta pasukan kesihatan bersekutu.

Awake craniotomy merupakan teknik pembedahan saraf yang maju di mana pesakit berada dalam keadaan sedar semasa fasa kritikal pembedahan. Kaedah ini membolehkan pakar bedah menilai fungsi penting otak seperti pertuturan dan pergerakan secara berterusan semasa pembuangan tumor, sekali gus membantu memaksimumkan pembuangan tumor sambil memelihara fungsi neurologi yang penting.

Pesakit yang didiagnosis dengan High-grade glioma telah menjalani prosedur ini dengan jayanya dan kekal stabil dari segi neurologi sepanjang pembedahan. Penilaian neurologi secara intraoperatif membolehkan pembuangan tumor dilakukan dengan lebih tepat sambil melindungi kawasan otak yang penting.

Pencapaian ini merupakan satu kemajuan besar dalam keupayaan perkhidmatan neurosurgeri di Kelantan, sekali gus mencerminkan kepakaran serta komitmen pasukan Neurosurgeri dalam menyediakan rawatan terkini kepada pesakit dengan tumor otak yang kompleks.

Kejayaan prosedur ini turut menonjolkan kepentingan kerjasama multidisiplin dan menjadi satu lagi langkah ke hadapan dalam memperluaskan perkhidmatan neurosurgeri berteknologi tinggi di bawah Kementerian Kesihatan Malaysia.

A Year of Neurosurgery Service : A Journey Worth Celebrating

Today marks a proud milestone, the inauguration of our Neurosurgery High Dependency Ward (NHDU) and 1 year of Neurosurgery Service in HSIP.

We are honoured that this historic event was officiated by the Deputy Director-General (Medicine), YBrs. Dato’ Indera Dr. Nor Azimi binti Yunus, whose presence inspired and strengthened our journey.

From One to Many Hopes
• This service began, and still continues today, with only one neurosurgeon, limited facilities, and countless challenges.
• Yet, through teamwork and perseverance, we cared for emergency and elective cases, trauma and non-trauma, across all ages.
• Behind each success were sleepless nights, sacrifices, prayers and the smiles of healed patients and grateful families.

To all who made this possible, from state health leadership to hospital management, department heads, nurses, MAs, PPKs, therapists, pharmacists, radiographers, lab teams and every visible or unsung staff member thank you.

To our patients and families, thank you for your trust.
To YBrs. Dato’ Indera Dr. Nor Azimi, your support means the world to us.

This is more than a ward , it is a milestone.

A New Chapter in Neurosurgery at HSIP
1st July 2025

Today marks another historic milestone for the Neurosurgery Unit at Hospital Sultan Ismail Petra (HSIP). A testament to how far we’ve come and a reflection of our commitment to advancing neurosurgical services in Kelantan.

This morning, we successfully carried out our first suspected case of high-grade glioma tumour debulking using a combination of cutting-edge technology:
✅ Operating Microscope for high-precision visualization
✅ Intraoperative Fluorescence Dye for tumour margin enhancement
✅ Image Guidance System (IGS) for real-time navigational accuracy

This case marks our first brain tumour resection with microscope + fluorescence dye + IGS, and more importantly, it signifies that Neurosurgery HSIP is now stepping into the era of advanced cranial surgery

On the same meaningful day, we also celebrate the official use of Ward 5B as our dedicated Neurosurgery High Dependency Unit (NHDU). This ward will now serve as a specialized post-operative care unit for neurosurgical patients equipped for close monitoring and recovery after major neurosurgical interventions.

Together, these achievements represent a double milestone, not just a clinical success in the operating theatre, but also a systemic improvement in how we care for neurosurgical patients beyond surgery.

We extend our deepest gratitude to the hospital director and all other departments who have supported this journey from day one. This is not just our success but it’s a shared victory for multidisciplinary teamwork and service development.

Today, we successfully performed the first-ever tumor case in HSIP—a historic moment for the Neurosurgery HSIP. This marks another important step forward as we continue to expand our services.

Our aim is to provide the people of Kelantan with timely interventions and early treatment options, closer to home.

We are deeply grateful for the continued support from other departments and hospitals, which has been instrumental in facilitating our progress since the inception of this service.

Perhaps next, we will take on vascular cases in HSIP—who knows what the future holds?

From 6th to 9th February 2025, the Neurosurgery Department of Hospital Sultanah Aminah (HSA) hosted a prestigious conference focused on neurosurgery perioperative care.

Day 1
featured workshops on “High-Speed Drill Techniques” and “Pain Workshop”. Our team participated in the High-Speed Drill workshop, where we learned the fundamentals of handling the instrument and progressed to hands-on sessions, practicing on lamb skulls, spines, and scapulae. We extend our sincere gratitude to the “Medtronic team”for their exceptional guidance and expertise.

Day 2
centered on “Image-Guided Surgery (IGS)”and “Endoscopic Techniques”, presented by “Brainlab”and “B.Braun”. This session delved into advanced technologies currently shaping neurosurgical practices. We had the opportunity to operate IGS machines and endoscopic instruments, gaining firsthand experience of the tools and techniques used by neurosurgeons.

Days 3 and 4
were dedicated to lectures and interactive sessions with sponsors. We attended insightful presentations and visited various exhibition booths to explore the latest innovations and technologies in neurosurgery. These interactions provided a deeper understanding of the advancements driving the field forward.

We would like to express our utmost appreciation to Neurosurgery HSA for organizing this remarkable biennial event. For our team, which is relatively new to the neurosurgery community, this conference was an eye-opening and invaluable experience.

We hope that, in the near future, Neurosurgery HSIP will be equipped with the latest instruments and technologies to enhance the quality of care for the people of Kelantan.

Once again, thank you to Neurosurgery HSA for this incredible opportunity. We look forward to future collaborations and hope to be invited to more events like this.
