Today marks a meaningful milestone for the Vascular Surgery fraternity in Malaysia.

We celebrate the retirement of two towering figures from Hospital Kuala Lumpur — Datuk Dr Hanif Hussein and Dr Kumaraguru V K Pillay whose dedication, mentorship, and leadership have shaped the landscape of vascular surgery in our country. Between them, they have trained nearly 70% of the current vascular surgeons in Malaysia, leaving behind a legacy that will continue to benefit countless patients for generations.

While they have officially retired, we are deeply grateful to the KEMENTERIAN KESIHATAN MALAYSIA for extending their service on contract, allowing them to continue contributing their invaluable expertise to Hospital Kuala Lumpur and the nation.

Their commitment goes far beyond clinical excellence — they have built a culture of discipline, compassion, and surgical mastery that lives on in all of us they have trained.

Thank you, Datuk Dr Hanif and Dr Kumaraguru, for your tireless service, mentorship, and inspiration. Your legacy is not just in what you have done, but in the many surgeons you have shaped.

HKL Vascular Team is feeling proud with Michael Arvind Karthigesu Abimanan Aizat Tamlikha Benjamin Leong Syazwan Arif Saprudin Brentha Kandasamy Najimudin Abdullah at Hospital Kuala Lumpur

A fantastic two days at the 6th HKL AV Fistula Workshop 2026 (7–8 April) at Hospital Kuala Lumpur!

Well-organised and thoughtfully executed by Dr Koh Pei Fern, this workshop brought together 10 general surgeons from across Malaysia for an intensive, hands-on experience in mastering arteriovenous fistula surgery.

This workshop reflects the continuous effort by the HKL Vascular Team to guide and expand vascular access creation and maintenance across the region. It is encouraging to see this initiative grow, with the hope that such efforts will continue to evolve beyond surgical training—towards strengthening maintenance and preservation of vascular access across all levels of healthcare.

From practical surgical sessions to meaningful discussions, the strong engagement and commitment shown by participants highlight a shared goal: improving vascular access care nationwide.

🎗️ Aortic Emergencies During Raya – A Wake-Up Call

Over the past few days during the Raya weekend, our team managed 7 aortic cases — 5 of them ruptured abdominal aortic aneurysms (AAA). These are among the most critical vascular emergencies we face, and we are grateful to have been able to save lives through urgent intervention. But this raises an important question: How common is ruptured AAA in Malaysia?

At Hospital Kuala Lumpur (HKL) — the largest vascular referral centre in the country — we performed 120 aortic surgeries in 2025. Alarmingly, 25% (30 cases) were ruptured AAAs.

Taking into account other centres across Malaysia (public, university, and private hospitals), we estimate ~100 ruptured AAA cases annually nationwide.

⚠️ Why is this concerning?

* Sudden and catastrophic
* High mortality
* Surgically challenging with significant morbidity

Most importantly, many of these cases could have been detected earlier.

🩺 The Solution: Early Screening Saves Lives

This is why HKL launched a pilot AAA screening programme in March 2025.

✔️ Free screening available
✔️ Non-invasive ultrasound
✔️ Walk-in on selected days

We strongly believe that early detection can prevent rupture, reduce emergency surgeries, and ultimately save lives.

This is a small but important step — and with continued support, we hope it can evolve into a national screening programme.

Not all aneurysms are the same - even when they are labelled as “infected.”

In simple terms, an infected aortic aneurysm is a serious condition where part of the main blood vessel in the body becomes weakened due to infection. It is rare, but potentially life-threatening.

In our recent study, we found that doctors and researchers around the world often use different ways to define and diagnose this condition. This means that cases grouped under the same name may actually be quite different.

Why does this matter?
Because if we are not comparing the same condition, it becomes harder to understand which treatments work best and what outcomes to expect.

We also looked at patient outcomes such as survival and need for further procedures. Interestingly, results were quite variable, which may partly be explained by these differences in diagnosis and classification.

This highlights an important point:
Better and more consistent definitions can lead to better research—and ultimately better care for patients.

A Demanding but Encouraging Day for Vascular Surgery HKL

The day began with the management of an emergency ruptured abdominal aortic aneurysm (AAA), followed by four elective aortic cases throughout the day:
🔹 One open repair for a juxtarenal abdominal aortic aneurysm
🔹 One TEVAR with in-situ fenestration for blunt thoracic aortic injury
🔹 Two EVARs for abdominal aortic aneurysms, including one performed using CO₂ as contrast.

Concurrently, the team also performed one lower limb angioplasty, one lower limb bypass, and several vascular access procedures, reflecting the breadth of vascular care provided.

This was made possible through the collective effort of a stable team of vascular surgeons, trainees, operating theatre staff, and, most importantly, our dedicated and enthusiastic vascular anaesthesia colleagues. It was truly a multidisciplinary effort across multiple clinical settings.

As the largest vascular referral centre in Malaysia, such days reflect the increasing demand placed on our service.
This reflects not merely procedural volume, but the capacity to deliver complex aortic care safely and consistently within a stable multidisciplinary framework.

We remain focused on steady improvement and sustainability, with continued support from the entire team, towards enhancing the overall quality of vascular surgery care for our patients.

The 3rd Northern AVF Workshop, Perlis

The Vascular Surgery Team from Hospital Kuala Lumpur (HKL) was pleased to travel north in support of the 3rd Northern AVF Workshop, held for the first time at Hospital Kangar, Perlis.
Our team was led by Dr Karthigesu Aimanan, with the workshop organised by Dr Muhammad Muslim, who has been instrumental in establishing and advancing AVF services in Perlis. The programme was further strengthened by the participation of Ahmad Firdaus Ahmad Lutfi, Dr Ashraf Md Razi and MA Abdul Rahim, who joined the workshop as speakers and shared their valuable expertise.
The workshop brought together young General Surgeons, focusing on strengthening foundational knowledge and enhancing technical skills in arteriovenous fistula (AVF) creation and management.
Held in Perlis, the northernmost state of Peninsular Malaysia, the session provided a valuable platform for collaboration, knowledge-sharing, and capacity building across the region.
From HKL, we are truly happy to support and contribute to the continued development of AVF services in the northern region. We look forward to more opportunities to work together in advancing vascular care for our patients.

4th Basic Endovascular Workshop, HKL

Our vascular trainees recently completed the 4th Basic Endovascular Workshop at Hospital Kuala Lumpur, gaining valuable hands-on experience in endovascular techniques using our simulation lab.

We were also joined by Artivion, Inc., Medtronic, BD, Inari Medical, and Boston Scientific, who showcased their latest endovascular technologies, giving our trainees great exposure to current innovations in vascular intervention.

Thank you to our industry partners and the dedicated HKL vascular team for making this workshop a continued success in advancing endovascular education and patient care in Malaysia.
