“Doc, can use glue only ah? No stitches 😅”

Actually not all cuts are suitable for skin glue.

For small and clean cuts, glue can work very well. Faster, less painful, and no need remove stitches.

But for deeper cuts, proper layered stitches usually heal better and give stronger support for nicer scar healing.

Treatment depends on the wound, not the trend.

Muka terseret atas jalan tar bukan sekadar “luka biasa”.

Harini aku rawat satu kes deep facial abrasion wound dengan beberapa tompokan hitam pada p**i & dahi akibat partikel tar/debris yang tertanam di bawah kulit.

Ramai tak tahu keadaan ini boleh menyebabkan traumatic tattooing - bila partikel hitam tertinggal dalam dermis lalu meninggalkan kesan seperti tattoo kekal selepas luka sembuh.

Sebab tu facial abrasion seperti ini bukan sekadar cuci luka biasa. Thorough wound cleaning & debridement sangat penting untuk mengurangkan risiko parut dan permanent tattooing effect.

Once the wound heals, kesan hitam tersebut menjadi jauh lebih sukar untuk dirawat even with laser treatment.

Early treatment matters, especially for facial injuries.

Keloid keeps coming back?

I often see patients with the same problem:
You’ve had injections - it goes down for a while, then grows again.
You’ve had surgery - looks better at first, but comes back.

This is actually common for stubborn (recurrent) keloids.

There are many ways to treat keloids - injections, surgery, creams, pressure therapy, tape etc. But when it keeps coming back, we may need a different approach.

In selected cases, one option is removal of the keloid followed by low-dose radiotherapy, done under oncologist’s supervision.

The goal of this low-dose radiation is to reduce the cells that cause the keloid to grow again.

Not everyone is suitable. It depends on the type, location & previous treatments.

If your keloid keeps coming back, it’s worth getting a proper assessment.

Dah cuba macam-macam...subcision, CO₂ laser, fillers, microneedling...tapi parut tak banyak berubah?

Tak semua parut sesuai dengan rawatan tanpa pembedahan.

Ada kes yang perlukan scar revision surgery untuk hasil yang lebih baik.

Petang ni aku buat scar revision.

Parut dah jadi widened & depressed. Patient dah try CO2 laser, subcision, microneedling but improvement limited.

At this stage, this is a structural defect

The only definitive option is surgical revision but not simple excision.

So aku guna W-plasty:
• Break the straight line
• Redistribute tension
• Make scar less noticeable

Setiap angle kena precise - planning & ex*****on

Concept nampak simple, but ex*****on actually complex.

End goal: bukan perfect skin, tapi scar yang less obvious & more acceptable long term.

Windscreen injury.
Not just cuts but shattered glass everywhere.

Face, scalp, limbs.
Severe tissue loss. Exposed skull.

3 hours overnight - exploring wounds, removing glass, reconstructing.
Finished at 2 AM.

But here’s the reality…
not all glass can be found during surgery.

Some fragments stay hidden.
They may surface later, form a lump (normal foreign body reaction),
or only show up on imaging.

That’s why this must be explained clearly from the start.

Explain early. Explain clearly.
To avoid misunderstanding & potential medicolegal issues later.
