Not all breathlessness is due to blocked heart arteries

A 58-year-old gentleman from Sibu came to us with worsening shortness of breath over the past 2 months.

He had a history of heart artery disease and had a stent placed in 2021. Naturally, the concern was that the stent might be blocked again.

But surprisingly — repeat angiograms showed his stent was completely clear.

So what was causing his symptoms?

Further tests revealed something different:
His ECG and Holter monitor showed frequent abnormal heartbeats called PVCs (premature ventricular contractions) — making up 31% of his total heartbeats.

These extra beats were coming from a specific area inside the heart called the posterior papillary muscle.

Medication (bisoprolol) was tried, but it didn’t control the problem.

👉 So we proceeded with a catheter ablation:
Using advanced 3D mapping and a special contact-force catheter, we identified and eliminated the exact spot causing the abnormal signals.

✅ Ablation time: 37 minutes
✅ Total procedure time: 2 hours
✅ Result: Successful elimination of the PVC source

💡 Why this matters:
Frequent PVCs like this can weaken the heart over time and lead to heart failure if left untreated.

Thankfully, this was diagnosed early and treated effectively — preventing long-term damage.

Take-home message:
Not all breathlessness means blocked arteries. Sometimes, the problem lies in the heart’s electrical system — and it can be treated.



气喘不一定是心脏血管阻塞引起的

一位来自诗巫的58岁男士，
在过去2个月里逐渐出现气喘加重的情况。

他本身有冠状动脉疾病病史，并在2021年接受过支架植入。
因此，最初大家都担心是否支架再次阻塞。

但令人意外的是——
重复进行的冠状动脉造影显示：支架完全通畅，没有阻塞。

那问题到底出在哪里？

进一步检查发现了不同的原因：
心电图（ECG）和24小时动态心电图（Holter）显示，他有频繁的异常心跳，称为室性早搏（PVC），占总心跳的31%。

这些异常电信号来自心脏内部一个特定结构：后乳头肌。

我们先尝试药物治疗（Bisoprolol），但效果不理想。

👉 因此，我们进行了导管消融手术：
通过先进的三维电生理定位系统和接触力导管，精准找到并消除产生异常电信号的部位。

✅ 消融时间：37分钟
✅ 总手术时间：2小时
✅ 结果：成功消除PVC来源

💡 为什么这很重要？
如果不治疗，频繁的PVC可能会逐渐削弱心脏功能，甚至导致心力衰竭。

幸运的是，这位患者被及时诊断并成功治疗，避免了长期的心脏损伤。

重点总结：
气喘不一定代表血管阻塞，有时问题出在心脏的“电系统”，而且是可以治疗的。
