Months before the actual transport, our team worked closely with our VVIP client through multiple discussions and planning sessions to ensure every detail was carefully coordinated ahead of time.

Last weekend, the patient successfully completed her treatment in Kuala Lumpur and was safely transferred back to Singapore.

For this mission, multiple components were coordinated to ensure comfort, safety, and seamless ex*****on — including ambulance arrangements, a dedicated VVIP MPV for accompanying family members, and a full medical es**rt team comprising a Doctor and Nurse.

Long journeys involve more than just moving a patient from one location to another. We also planned suitable midway rest stops for the family and entourage, allowing them time for a short meal break and to refresh during the cross-border journey.

Behind every transfer is extensive preparation, coordination, and teamwork to ensure both the patient's medical needs and the family's concerns are looked after.

Sometimes, it starts with a phone call like this:

"Hi Mr Ong, I am a Medical Social Worker calling from the hospital. I have a Malaysian patient currently admitted in the Neuro ICU. Due to financial constraints, the family is requesting a transfer back to a hospital in Malaysia for continuation of care."

"Sure, we can coordinate this with a medical team. Just checking — is the patient currently under the care of the Trauma Services?" I replied.

"Yes, the hospital's Trauma Services team will be liaising with their Malaysian counterpart."

For this particular hospital, the Trauma Services team plays a vital role in coordinating cross-border medical repatriation cases with their counterparts in Malaysia.

This week was a challenging one.

ICU beds were extremely limited, not just in one hospital but across multiple facilities in the area. Every call, update and coordination mattered because every additional day of waiting meant increasing stress for the family and mounting medical expenses for a patient receiving treatment overseas.

After days of continuous discussions and coordination between hospital teams, trauma services, social workers and receiving facilities, we eventually managed to transfer the patient back safely.

It was never simply about arranging transport.

Many do not see the countless calls behind the scenes, the after-office-hour discussions, the coordination between different healthcare teams, or the continuous problem-solving required to move a critically ill patient safely across borders.

Every cross-border medical repatriation comes with its own set of challenges.

As a medical assistance provider, our role extends beyond transportation. We look at the bigger picture — identifying barriers, finding solutions and coordinating every moving piece with one common goal: bringing the patient home safely and as early as possible.

“Hi! We have a patient who wants to be transferred back to Singapore now. He is currently in our ED for MI.”

“MI? Confirmed STEMI?” I replied immediately.

“Yes. STEMI on ECG and Troponin I affirms the indication.”

“How is the patient now?”

“He is conscious, on supplementary oxygen.”

MI — Myocardial Infarction. To many, it is known simply as a heart attack.

Back during my nursing and paramedic training days, one phrase was constantly drilled into us:
“Time is muscle.”

Every minute delayed means more heart muscle being damaged.

The moment the call came in, coordination work immediately started. I began checking available resources and ambulance teams while liaising closely with the hospital. Fortunately, the hospital activated their contracted ambulance provider, which helped save valuable time.

What many people do not see behind urgent cross-border transfers are the countless moving parts happening simultaneously behind the scenes.

Even while standing outside a shop in the middle of handling my own errands, I spent the next 30 minutes coordinating crews, equipment, escalation plans, and receiving arrangements.

For this particular case, we specifically requested:
• A paramedic onboard
• Emergency cardiac medications
• A patient monitor capable of 12-Lead ECG monitoring
• Pre-alert coordination on both sides of the border

A STEMI transfer is never “just transport.”

Every provider involved had to be carefully scrutinised because if the case is not handled properly, the patient’s condition could deteriorate rapidly during transfer.

I eventually informed the team:
“I will personally es**rt this case as well.”

Because in situations like this, having another trained pair of hands onboard can make all the difference.

Upon arrival at the Malaysia checkpoint, the patient was handed over by the hospital’s es**rting doctor.

The moment the patient came onboard, our clinical work started immediately.

A repeat 12-Lead ECG was performed inside the ambulance.

Five letters appeared once again on the screen:
“STEMI.”

At that point, we prepared ourselves mentally for the worst-case scenario and immediately initiated pre-notification to both the Singapore checkpoint and Changi General Hospital for a STEMI alert activation.

Thankfully, despite the late hour, traffic conditions were smooth.

Within approximately 15 minutes, we arrived safely at Changi General Hospital’s Emergency Department.

As the crews transferred the patient directly to the awaiting medical team, I simultaneously assisted with the registration process so treatment could commence without unnecessary delay.

Cases like this are reminders that urgent medical evacuations should never be viewed as mere transportation.

It is a clinically supported mission where every decision, every crew member, every piece of equipment, and every minute matters.

Arranging an ambulance is easy.

Coordinating the right medical team, clinical preparedness, operational support, and cross-border logistics for a patient actively suffering a heart attack — that is where our responsibility truly begins.

These are never simple cases.

A cardiac arrest is not just a medical emergency. It is a race against time, involving critical care, specialist coordination, and seamless communication between hospitals across two countries.

Mr T suffered a cardiac arrest while on his way to a GP clinic after experiencing chest pain. His daughter found him unresponsive, without a palpable pulse, and immediately called the Emergency Medical Services from the Singapore Civil Defence Force.

While waiting for the ambulance team to arrive, she began CPR and delivered one shock using a nearby public access defibrillator.

The emergency team continued resuscitation efforts, and Mr T achieved Return of Spontaneous Circulation (ROSC). However, his condition remained extremely critical. He arrested again during transport, once more upon arrival at the Emergency Department, and another time inside the resuscitation bay.

After successful resuscitation, he was transferred from Woodlands Hospital to Khoo Teck Puat Hospital under intensive care management.

Doctors later confirmed an anterior STEMI as the cause of his cardiac arrest. Complications soon followed — including cardiogenic shock and septic shock.

After nearly a week in the ICU in Singapore, the family faced a difficult decision. Due to financial constraints and the wish for him to continue treatment closer to home in Malaysia, they decided on medical repatriation.

From that point, every hour mattered.
