[👴 MPS-YPC Compassion In Care: Old Folks Home Visitation 2026 👵]
Small Acts of Care Make A Big Difference! ❤️

We recently spent a meaningful day of service at two old folks' homes—The Association of Klang and Coast Chik Sin Thong Old Folks Home & Pertubuhan Kebajikan Warga Emas Sg Chua for our Medication Reconciliation Outreach. 💊✨

During our visits, our team supported the residents by:
📝 Reviewing medication lists to improve compliance and reduce errors.
🔍 Identifying risks like polypharmacy, drug interactions, incorrect device technique and expired medicines.
📊 Providing basic health screenings to promote long-term wellness.

🤝 A heartfelt thank you to all our dedicated volunteers, committee members, Big Caring Group and the homes for making this outreach a huge success. Together, we are building a healthier, safer community!
