Then you remember:

Your hand moves towards the sink.

Stop.

Taking medicine does not automatically make your breast milk unsafe—or mean breastfeeding must stop.

But the medicine needs to be checked properly.

Paracetamol and ibuprofen are generally compatible with breastfeeding when used appropriately. However, not every painkiller is the same.

Cough and cold products may contain several active ingredients. Some may cause drowsiness, while pseudoephedrine may reduce milk production in some mothers.

Herbal products also require checking.

Record these five things:

Note the colour, consistency and frequency.

Is your baby generally settled, or repeatedly distressed during or after feeding?

🤮 𝐒𝐏𝐈𝐓-𝐔𝐏 𝐎𝐑 𝐕𝐎𝐌𝐈𝐓𝐈𝐍𝐆
Occasional spit-up can happen. Watch for repeated vomiting or a significant new pattern.

👶 𝐒𝐊𝐈𝐍 & 𝐎𝐕𝐄𝐑𝐀𝐋𝐋 𝐖𝐄𝐋𝐋𝐁𝐄𝐈𝐍𝐆
Look for a new or worsening rash, swelling, breathing difficulty, unusual sleepiness or weakness.

Do not wait seven days if warning signs appear.

She was coughing.

Her nose was blocked.

Then the headache started.

So she opened the medicine cabinet.

One medicine for the cough and cold.

Another tablet for the headache.

Different boxes.

Different names.

Different symptoms printed on the front.

She thought she was taking two different medicines.

Then the pharmacist turned both boxes around.

The first product contained:

𝐏𝐚𝐫𝐚𝐜𝐞𝐭𝐚𝐦𝐨𝐥.

The second product contained:

𝐏𝐚𝐫𝐚𝐜𝐞𝐭𝐚𝐦𝐨𝐥—𝐚𝐠𝐚𝐢𝐧.

Suddenly, the problem was no longer just her cough or headache.

𝐒𝐡𝐞 𝐦𝐚𝐲 𝐡𝐚𝐯𝐞 𝐭𝐚𝐤𝐞𝐧 𝐭𝐡𝐞 𝐬𝐚𝐦𝐞 𝐚𝐜𝐭𝐢𝐯𝐞 𝐢𝐧𝐠𝐫𝐞𝐝𝐢𝐞𝐧𝐭 𝐭𝐰𝐢𝐜𝐞 𝐰𝐢𝐭𝐡𝐨𝐮𝐭 𝐫𝐞𝐚𝐥𝐢𝐬𝐢𝐧𝐠 𝐢𝐭.


This happens more easily than many people think.

Cough, cold and flu medicines are often 𝐜𝐨𝐦𝐛𝐢𝐧𝐚𝐭𝐢𝐨𝐧 𝐩𝐫𝐨𝐝𝐮𝐜𝐭𝐬.

One product may contain several active ingredients to relieve:

Fever or pain.

Cough.

Runny nose.

Allergy symptoms.

Nasal congestion.

Then, because the headache has not improved, another painkiller is added.

You may think:

“𝐓𝐡𝐢𝐬 𝐨𝐧𝐞 𝐢𝐬 𝐟𝐨𝐫 𝐦𝐲 𝐜𝐨𝐮𝐠𝐡.”

“𝐓𝐡𝐚𝐭 𝐨𝐧𝐞 𝐢𝐬 𝐟𝐨𝐫 𝐦𝐲 𝐡𝐞𝐚𝐝𝐚𝐜𝐡𝐞.”

But your body does not read the name on the front of the box.

It receives every active ingredient inside.


The brand may be different.

The packaging may be a different colour.

One may be a syrup.

The other may be a tablet.

But the same ingredient may still appear in both.

That is how accidental duplication happens.

Not because someone wanted to take too much.

But because nobody realised they were counting the same ingredient twice.


That is why your pharmacist turns the box around.

We are not only looking at the large words printed on the front:

We are looking for the smaller words on the back:

That small section may tell you more about the medicine than everything promised on the front.


And if you are pregnant, checking for duplication is only the first step.

Even when no ingredient appears twice, it does not automatically mean the product is appropriate for you.

We still need to know:

How many weeks pregnant are you?

What symptoms do you actually have?

What ingredients are inside?

What dose are you taking?

What other medicines or supplements are you using?

Do you need every ingredient in that combination product?

A medicine made for several symptoms may contain ingredients you do not need.

And during pregnancy, every active ingredient should be considered—not only the one you recognise.


Before taking two cough, cold, flu, allergy or pain medicines together:

🔍 Turn both packages around.

🔍 Find the 𝐚𝐜𝐭𝐢𝐯𝐞 𝐢𝐧𝐠𝐫𝐞𝐝𝐢𝐞𝐧𝐭𝐬.

🔍 Compare them line by line.

🔍 Check whether anything appears twice.

🔍 If you are pregnant or uncertain, show both products to your pharmacist or healthcare professional before taking them together.

Do not compare only the brand names.

Do not judge by the colour of the packaging.

And do not assume:

Then she remembered the iron tablets she had recently started taking.

Since then, her stomach had felt unsettled.

Nausea.

Constipation.

Sometimes, stomach discomfort.

And now this.

Her stool looked almost black.

Her first thought was immediate:

“𝐓𝐡𝐞𝐬𝐞 𝐭𝐚𝐛𝐥𝐞𝐭𝐬 𝐝𝐨𝐧’𝐭 𝐬𝐮𝐢𝐭 𝐦𝐞.”

“𝐌𝐚𝐲𝐛𝐞 𝐈 𝐬𝐡𝐨𝐮𝐥𝐝 𝐬𝐭𝐨𝐩.”


But dark or black-looking stool can be an expected effect of oral iron.

Iron tablets may also cause nausea, constipation and stomach discomfort.

So, if your stool becomes darker after starting iron, it does not automatically mean that something dangerous has happened.

But there is something important every mother should know:

𝐈𝐫𝐨𝐧 𝐜𝐚𝐧 𝐝𝐚𝐫𝐤𝐞𝐧 𝐲𝐨𝐮𝐫 𝐬𝐭𝐨𝐨𝐥.
𝐁𝐮𝐭 𝐧𝐨𝐭 𝐞𝐯𝐞𝐫𝐲 𝐛𝐥𝐚𝐜𝐤 𝐬𝐭𝐨𝐨𝐥 𝐬𝐡𝐨𝐮𝐥𝐝 𝐛𝐞 𝐛𝐥𝐚𝐦𝐞𝐝 𝐨𝐧 𝐢𝐫𝐨𝐧.


Colour alone does not tell the whole story.

Black stool caused by gastrointestinal bleeding may look unusually sticky or tar-like.

It may also be accompanied by:

🚩 Stomach pain
🚩 Dizziness or fainting
🚩 Unusual weakness
🚩 Visible blood
🚩 Vomiting blood or material resembling coffee grounds

If this happens, don’t reassure yourself with:

“𝐈𝐭’𝐬 𝐨𝐧𝐥𝐲 𝐭𝐡𝐞 𝐢𝐫𝐨𝐧.”

Please seek medical assessment promptly.


But what if the problem is mainly nausea?

Every time you see the tablet, you already feel like vomiting.

So you skip one dose.

Then another.

One day becomes two.

Two days become a week.

Not because you don’t care.

But because something meant to support your pregnancy has become difficult to tolerate.

𝐏𝐥𝐞𝐚𝐬𝐞 𝐝𝐨𝐧’𝐭 𝐬𝐭𝐫𝐮𝐠𝐠𝐥𝐞 𝐪𝐮𝐢𝐞𝐭𝐥𝐲.

And don’t simply stop without telling anyone.

If iron was recommended because your body needs it, stopping treatment may leave that need unaddressed.

Tell your pharmacist or doctor:

What product are you taking?

How much are you taking?

When did the symptoms begin?

How severe are they?

Can you still eat and drink normally?

Your healthcare professional can review the iron preparation, dose and reason for treatment—and decide whether your routine needs to be adjusted or another approach should be considered.


Perhaps another mother tells you:

But iron products are not all the same.

They may contain different forms and amounts of iron.

And every mother’s needs are different.

𝐀𝐧𝐨𝐭𝐡𝐞𝐫 𝐦𝐨𝐭𝐡𝐞𝐫’𝐬 𝐢𝐫𝐨𝐧 𝐫𝐨𝐮𝐭𝐢𝐧𝐞 𝐢𝐬 𝐧𝐨𝐭 𝐲𝐨𝐮𝐫 𝐩𝐫𝐞𝐬𝐜𝐫𝐢𝐩𝐭𝐢𝐨𝐧.


The goal is not simply to make you finish a bottle of tablets.

The goal is to meet your iron needs in a way that is appropriate and tolerable—while recognising symptoms that should never be dismissed.

So, if iron changes your stool or unsettles your stomach, don’t make the decision based on fear alone.

Describe what changed.

When it started.

What the stool looks and feels like.

And which iron product you are taking.

Because sometimes, the symptom is expected.

Sometimes, the treatment needs to be reviewed.

And sometimes, your body is asking for medical attention.
