POST 9 — THE BERKAT APPROACH

ASSESS → CLASSIFY → PRESCRIBE → PROGRESS → REASSESS

This is the foundation of our new programme.

1️⃣ ASSESS

We establish your baseline.

2️⃣ CLASSIFY

Healthy & Active
Healthy but Not Active
Not Healthy & Not Active

3️⃣ PRESCRIBE

We determine the appropriate exercise and level of challenge.

4️⃣ PROGRESS

As your strength, balance and mobility improve, the programme progresses.

5️⃣ REASSESS

We measure whether meaningful changes have occurred.

This approach reflects a central principle of evidence-informed fall prevention:

Exercise should be individualized, functional and progressively challenging.

We have spent years learning from geriatric rehabilitation and from the patients we see every day.

And we have taken what we have learned and developed it into a practical, structured and clinically guided 12-week programme.

Not complicated.

Not one-size-fits-all.

Just a clear pathway towards better function.

POST 8 — YOUR STARTING POINT DOESN'T DEFINE YOUR DESTINATION

WHICH ONE ARE YOU?

🟢 HEALTHY & ACTIVE

“I exercise. I walk. I feel independent.”

Great.

Let's help you maintain it.

---

🟡 **HEALTHY BUT NOT ACTIVE

“I know I should exercise… but I just don't.”

You're not alone.

Let's help you **start safely and consistently.**

---

🔴 NOT HEALTHY & NOT ACTIVE

“I've become weaker. I don't move much anymore. I'm afraid I might fall.”

This is where individualized clinical guidance becomes particularly important.

Let's first understand why you're struggling and what can safely be improved.

There is no shame in where you are starting.

The important question is:

Where do you want to go from here?

Our initial consultation helps us determine your current functional status and the appropriate level of intervention.

You don't have to be fit to start.

You just need to start at the right level.

POST 7 — WHAT DOES “HEALTHY AGEING” REALLY MEAN?

IT'S NOT ABOUT LOOKING YOUNGER.

Healthy ageing is not about pretending that we are 30 again.

It's about being able to continue doing the things that matter.

Can you…

🛏️ Get out of bed independently?

🪑 Get up from a chair?

🚶 Walk to the shops?

🛒 Carry your groceries?

🪜 Climb the stairs?

👨‍👩‍👧 Spend time with your grandchildren?

⛪ Attend church?

✈️ Travel?

🌳 Enjoy an evening walk?

❤️ Live independently?

These are not small things.

These are the things that make life meaningful.

WHO describes healthy ageing in terms of maintaining the functional ability that enables wellbeing in older age.

So our programme isn't simply about:

“How many exercises can you do?”

It is about:

“What can you continue doing with your life?”

That is the kind of healthy ageing we want to promote.

🌿 Stay Active. Stay Independent. Stay You.

# POST 6 — THIS IS NOT JUST AN EXERCISE CLASS

# # # **WHAT MAKES OUR PROGRAMME DIFFERENT?**

You may have seen exercise classes for seniors.

Chair exercises.

Group stretching.

Walking groups.

General fitness programmes.

These can all have their place.

But our programme is designed differently.

# # # **THIS IS A PHYSIOTHERAPY-LED PROGRAMME.**

That means we begin with assessment.

We consider:

🧓 Your age and functional capacity
🩺 Your health conditions
⚖️ Your balance
💪 Your strength
🚶 Your mobility
🧠 Your confidence with movement
📋 Your history of falls
🎯 Your personal goals

Then we determine an appropriate starting level.

And as you improve…

# # # **we progress the challenge.**

Because an exercise that was difficult in Week 1 may become easy by Week 6.

And when your ability changes…

**your programme should change too.**

Current fall-prevention guidelines emphasize individualized, supervised and progressively challenging exercise rather than applying one standardized exercise programme to everyone.

# # # **We don't simply count repetitions.**

We measure whether those exercises are helping you function better.

POST 5 — WHY 12 WEEKS?

CAN YOU REALLY CHANGE YOUR FUNCTION IN 12 WEEKS?

We often hear:

> “Why 12 weeks?”

Because meaningful physical change doesn't happen overnight.

Strength needs to be developed.

Balance needs to be challenged.

Movement needs to be practised.

Confidence needs to be rebuilt.

And the body needs time to adapt.

Our programme therefore follows a simple clinical cycle:

WEEK 1

🧪 BASELINE ASSESSMENT

Where are you starting from?

WEEKS 2–5

🏋️ BUILD

Develop strength, balance and mobility.

WEEK 6

📈 **PROGRESS CHECK**

Are we moving in the right direction?

WEEKS 7–11

🚀 PROGRESS & CHALLENGE

Exercises are adjusted according to progress.

WEEK 12

🏁 FINAL ASSESSMENT

What has changed?

What still needs work?

Where do we go from here?

The international World Guidelines for Falls Prevention recommend individualized and progressive exercise programmes lasting at least 12 weeks, incorporating balance and functional exercises.

12 weeks isn't the finish line.

It is a structured period to build a stronger foundation for the years ahead.

POST 4 — THREE THINGS WE DON'T WANT TO LOSE AS WE AGE

⚖️ BALANCE

💪 STRENGTH

🚶 MOBILITY

Think about everyday life.

Getting out of bed.

Standing from a chair.

Walking to the bathroom.

Climbing stairs.

Reaching for something on a shelf.

Getting in and out of a car.

Going shopping.

All of these require a combination of strength, balance and mobility.

As we age, physical inactivity and declining physical capacity can make everyday activities progressively more difficult.

That is why our programme focuses on these three pillars.

⚖️ BALANCE

Because being able to control your body while standing and moving is fundamental to safe mobility.

💪 STRENGTH

Because everyday independence requires strength—not just in the gym, but when standing, walking, lifting and transferring.

🚶 MOBILITY

Because movement is what allows us to participate in life.

WHO recommends that older adults include varied multicomponent physical activity emphasizing functional balance and strength to enhance functional capacity and help prevent falls.

And international fall-prevention guidelines recommend individualized balance and functional exercise such as sit-to-stand, stepping and walking, combined where appropriate with progressive strength training.

We aren't training older adults to become athletes.

We're training them to remain capable of living their lives.

That is the difference.

POST 3 — BEFORE WE PRESCRIBE, WE ASSESS

“JUST EXERCISE” IS NOT A COMPLETE PRESCRIPTION.

When it comes to older adults, we believe exercise should be appropriate, progressive and purposeful.

Before starting our programme, participants undergo an initial consultation and functional screening.

We want to know:

🔹 How well do you move?

🔹 How strong are you?

🔹 How is your balance?

🔹 Have you fallen before?

🔹 Are you afraid of falling?

🔹 How easily can you get up from a chair?

🔹 How quickly and safely can you walk?

🔹 What are your current limitations?

We use practical clinical measures including:

🧪 Clinical Frailty Scale

To help characterize overall frailty and vulnerability.

⏱️ Timed Up & Go

To assess functional mobility.

🪑 5 Times Sit-to-Stand

To assess functional lower-limb strength and movement performance.

📋 History of Falls

Because previous falls are an important part of understanding future fall risk.

This approach is consistent with established fall-risk assessment frameworks. For example, the CDC's STEADI approach incorporates assessment of gait, strength and balance, along with fall history and other modifiable risk factors.

We don't guess where you need to start.

We assess it.

Then we build the programme around you.

Next: Why balance, strength and mobility?

POST 2 — AGEING DOESN'T LOOK THE SAME FOR EVERYONE

TWO PEOPLE. SAME AGE. COMPLETELY DIFFERENT ABILITIES.

Meet two imaginary friends.

👴 Mr. A — 70 years old

He walks every morning.

He gardens.

He goes shopping.

He attends church.

He exercises regularly.

He is confident on his feet.

Now meet:

👴 Mr. B — 70 years old

He spends most of his day sitting.

He rarely exercises.

He avoids stairs.

He feels weaker than he used to.

He is becoming less confident when walking outside.

They are the same age.

But do they need the same physiotherapy programme?

Probably not.

This is one of the reasons we designed our new Geriatric Healthy Ageing & Fall Prevention Programme around **individual assessment**.

We broadly categorize participants into three starting groups:

🟢 HEALTHY & ACTIVE

Already active and independent.

Focus: Maintain and challenge existing physical capacity.

🟡 HEALTHY BUT NOT ACTIVE

Generally healthy, but increasingly sedentary.

Focus: Safely rebuild strength, mobility, balance and confidence.

🔴 **NOT HEALTHY & NOT ACTIVE**

Reduced mobility, weakness, previous falls or multiple health concerns.

Focus: Establish safety and gradually improve functional capacity.

Same age does not mean same needs.

And that is why our programme doesn't begin with:

“Here are your exercises.”

It begins with:

“Let's understand where you are first.”

Next: What exactly do we assess?

POST 1 — THE QUESTION THAT STARTED IT ALL

WHAT IF WE DIDN'T WAIT FOR THE FALL?

A fall can change everything.

One moment, Dad is walking around the house independently.

Then one day…

He falls.

Suddenly there is a fracture.

Hospitalisation.

Fear of walking again.

Loss of confidence.

And sometimes, a long road back to independence.

But here's the question we want to ask:

Does physiotherapy have to begin AFTER the fall?

What if we started earlier?

Before the first fall.

Before significant weakness develops.

Before walking becomes difficult.

Before someone becomes afraid to move.

At BerkatPhysiotherapy Centre, years of working with older adults have taught us something important:

Prevention and early intervention matter.

Healthy ageing is not simply about living longer.

It is about maintaining the ability to move, function and participate in the things that matter to you.

That is why we have developed something new.

🌿 GERIATRIC HEALTHY AGEING & FALL PREVENTION PROGRAMME

A structured 12-week physiotherapy programme designed around:

⚖️ Balance
💪 Strength
🚶 Mobility
🛡️ Fall prevention
❤️ Independence

But there is something different about our programme.

We don't put everyone into the same exercise group.

First, we assess.

Then, we understand.

Then, we prescribe.

Coming soon…

We will show you why our first step isn't exercise.

Our first step is understanding YOU.
