Antara perkara penting yang perlu dinilai ialah:
1️⃣ Umur dan simpanan telur
Umur dan ovarian reserve penting kerana masa memainkan peranan dalam perancangan rawatan kesuburan.
2️⃣ Lokasi endometriosis
Endometriosis boleh menyebabkan parut atau lekatan yang mengganggu hubungan antara ovari dan tiub fallopio.
3️⃣ Pasangan juga penting
Penilaian kesuburan perlu melibatkan kedua-dua pasangan. Endometriosis mungkin salah satu faktor, tetapi bukan semestinya satu-satunya punca.
4️⃣ Pilih laluan yang sesuai
Bergantung pada keadaan individu, pelan mungkin melibatkan mencuba secara semula jadi, pembedahan dalam kes terpilih, atau rawatan kesuburan seperti IVF.

Rawatan hormon boleh membantu mengawal simptom endometriosis, tetapi ia tidak meningkatkan peluang kehamilan secara semula jadi semasa anda mengambilnya.

When someone with endometriosis is trying to conceive, the decision is not just about treating pain. We also need to look at the full fertility picture.

Important factors include:
1️⃣ Age and ovarian reserve
Egg reserve naturally changes with age, so timing can influence the next step.
2️⃣ Where the endometriosis is
Scarring or adhesions may affect the relationship between the ovaries and fallopian tubes.
3️⃣ Your partner’s fertility too
Fertility assessment should include both partners. Endometriosis may be one factor, but it may not be the only one.
4️⃣ Choosing the right pathway
Depending on the situation, this may involve trying naturally, surgery in selected cases, or fertility treatment such as IVF.

Hormonal treatment can be useful for controlling endometriosis symptoms, but it does not improve natural pregnancy chances while you are taking it.

If pregnancy is part of your future plan, especially if you are already having difficulty conceiving, it is worth discussing fertility early with your gynaecologist.

The right approach should always be personalised to your age, symptoms, fertility goals, ovarian reserve, disease pattern, and your partner’s assessment.

𝗖𝗮𝗻 𝗡𝗔𝗖 𝗿𝗲𝗮𝗹𝗹𝘆 𝗵𝗲𝗹𝗽 𝘄𝗶𝘁𝗵 𝗲𝗻𝗱𝗼𝗺𝗲𝘁𝗿𝗶𝗼𝘀𝗶𝘀?
N-acetylcysteine, or NAC, has gained attention because of its antioxidant and anti-inflammatory properties, and researchers are exploring whether it may have a supportive role in endometriosis.

One prospective study involving 120 women with endometriosis looked at NAC taken for 3 months. The regimen used was 600 mg, 3 tablets per day, for 3 days each week.

After the intervention, researchers reported improvements in:
• period pain, pain during in*******se and chronic pelvic pain
• use of NSAID painkillers
• average endometrioma size, which decreased slightly
• pregnancy outcomes among some women who were trying to conceive

These findings are interesting, but they need to be interpreted carefully.

This study was not a randomized placebo-controlled trial, which means we cannot say for certain that NAC itself caused these improvements. Pregnancy outcomes are especially important to interpret cautiously, as many other factors can influence fertility.

Other studies have also produced mixed results, and at this point NAC is not considered an established standard treatment for endometriosis.

So, is NAC promising? Possibly.

Is it proven? Not yet.

The most important thing is that supplements should not replace proper assessment, imaging, medical treatment or surgery when these are needed.

If you are considering NAC, particularly if you are trying to conceive or taking other medications, discuss it with your gynaecologist first so it can be considered in the context of your overall treatment plan.

Research is constantly evolving, and part of good endometriosis care is understanding both the potential benefits and the limitations of the evidence.

Keputusan scan yang normal tidak semestinya bermaksud tiada endometriosis.
Endometriosis bukan selalu kelihatan sebagai ketulan besar. Ia juga boleh menyebabkan perubahan yang lebih halus seperti parut, tisu yang menebal atau organ yang melekat.
Scan rutin mungkin lebih mudah mengesan sista atau masalah yang jelas, tetapi tanda endometriosis yang halus boleh terlepas pandang.
Sebab itu, pengalaman dalam melakukan dan menilai pemeriksaan untuk endometriosis adalah penting.
Jika anda masih mengalami sakit pelvis, sakit haid atau sakit ketika membuang air besar walaupun scan normal, pertimbangkan untuk mendapatkan pemeriksaan lanjut.

A normal scan doesn’t always mean you don’t have endometriosis.
Endometriosis doesn’t always appear as a large, obvious lump. It can also cause subtle changes such as scarring, thickened tissue or organs becoming stuck together.
Routine scans may be better at identifying cysts or more obvious problems, while subtle signs of endometriosis can sometimes be missed.
That’s why experience in performing and interpreting imaging for endometriosis matters.
If you’re still experiencing pelvic pain, painful periods or pain when passing motion despite a normal scan, consider further assessment.

Inflammation and adhesions can cause organs such as the ovaries, fallopian tubes, bowel or bladder to become stuck together or pulled out of their usual position. This may affect how the organs move and function, and can contribute to pain, bowel or bladder symptoms, painful in*******se, and sometimes fertility concerns.
This is also why understanding where the disease is and how much the anatomy has been affected is so important when planning treatment or surgery.

Endometriosis rektum berlaku apabila endometriosis yang dalam melibatkan rektum. Simptom boleh termasuk sakit ketika buang air besar, sembelit, kembung perut, sakit pelvis yang dalam dan sakit ketika bersama.
Satu petunjuk penting ialah masa simptom berlaku. Jika simptom ini berulang kali menjadi lebih teruk ketika haid, ia wajar dibincangkan dengan doktor.
Namun, simptom usus sahaja tidak bermaksud anda mempunyai endometriosis pada usus. Pemeriksaan yang sesuai, termasuk ultrasound khusus atau MRI jika diperlukan, boleh membantu menentukan sama ada usus terlibat dan sejauh mana penyakit tersebut.

Jika simptom anda kerap berulang ketika haid, berjumpalah dengan Pakar Sakit Puan yang berpengalaman dalam rawatan endometriosis.

Re**al endometriosis occurs when deep endometriosis involves the re**um. Symptoms can include painful bowel movements, constipation, bloating, deep pelvic pain and pain during in*******se.
One important clue is timing. If these symptoms repeatedly become worse around your period, it’s worth discussing them with a doctor.
However, bowel symptoms alone do not mean you have bowel endometriosis. Proper assessment, including specialised ultrasound or MRI when appropriate, can help determine whether the bowel is involved and how deep the disease goes.
