Blood Transfusion Science (Immunohematology): An Overview
What is Transfusion Medicine?
Blood Transfusion Science, also known as Immunohematology, is a specialized branch of Medical Laboratory Science concerned with the collection, testing, processing, storage, compatibility assessment, and safe transfusion of blood and its components.
Its primary objective is to ensure that the appropriate blood component is administered to the appropriate patient at the appropriate time, thereby maximizing therapeutic benefit while minimizing transfusion-related risks.
Clinical Importance of Blood Transfusion
Blood transfusion is a critical, life-saving intervention used in the management of patients with:
Severe hemorrhage (e.g., trauma or postpartum hemorrhage)
Severe anemia
Sickle cell disease and other hemoglobinopathies
Major surgical procedures
Malignancies and hematological disorders
Major Blood Components and Their Clinical Uses
Packed Red Blood Cells (PRBCs): Primarily indicated for the treatment of significant anemia and acute blood loss to improve oxygen-carrying capacity.
Platelets: Used to manage thrombocytopenia or platelet dysfunction and to prevent or control bleeding.
Fresh Frozen Plasma (FFP): Contains essential coagulation factors and is administered to correct clotting factor deficiencies in selected bleeding disorders.
Cryoprecipitate: A concentrated source of fibrinogen, Factor VIII, von Willebrand factor, and Factor XIII, used in specific coagulation disorders, particularly hypofibrinogenemia.
Blood Group Systems
The two most clinically significant blood group systems are:
ABO Blood Group System: A, B, AB, and O.
Rhesus (Rh) Blood Group System: Rh-positive (Rh⁺) and Rh-negative (Rh⁻).
Accurate determination of both ABO and Rh blood groups is fundamental to safe transfusion practice.
