"His PCV is at 18. The reticulocyte count is elevated. HbF levels are here. WBC is slightly raised."

— sound like a completely different language. 😔

And you want to ask questions. You really do. But you don't know where to start because you don't even know what you don't know. So you collect the papers. You say thank you. You walk out.

And in the car you look at those numbers and wonder —

"Is this good? Is this bad? Should I be worried? Is my child okay?"

If this has ever been you — and I suspect it has been most of us at some point — this post is long overdue. 💙

Because here is the truth nobody says loudly enough —

Understanding your child's blood results is not the doctor's job alone. It is yours too. And you deserve to walk into every appointment and out of every laboratory with the confidence of someone who knows what they're looking at.

So let's break it down. Simply. Clearly. No medical grammar. 👇🏾

The Blood Test Most Sickle Cell Patients Do Regularly Is Called A Full Blood Count — FBC

Think of it as a complete report card of what is happening inside the blood right now. It looks at several things simultaneously and gives the doctor — and now YOU — a picture of how the body is coping.

Here are the key numbers and what they actually mean —

1. PCV — Packed Cell Volume
(Also called Haematocrit)

🔬 What it measures: The percentage of your blood that is made up of red blood cells.

📊 Normal range for sickle cell patients:

Children — roughly 20% to 30%
Adults — roughly 20% to 35%

(Note — these are lower than the ranges for people without sickle cell. SS patients run at a naturally lower baseline. This is expected.)

✅ What a good result looks like: Stable. Consistent with their own personal baseline. Not dramatically lower than their last result.

🚨 What to worry about: A sudden DROP from their usual baseline. If your child normally runs at 24% and today's result is 16% — that is significant and needs immediate attention.

The key word here is BASELINE. Every sickle cell patient has their own normal. Learn your child's number when they are well. That number becomes your reference point for everything else. 💙

2. HGB — Haemoglobin
(Written as Hb or HGB on the report)

🔬 What it measures: The protein inside red blood cells that carries oxygen around the body. This is THE central number in sickle cell management.

📊 Typical range for SS patients:

Children — roughly 6 to 9 g/dL
Adults — roughly 6 to 10 g/dL

(Again — lower than non-sickle cell ranges. This is expected for SS.)

✅ What a good result looks like: Stable. Close to their personal baseline.

🚨 What to worry about: Anything significantly below their usual baseline. Symptoms like extreme fatigue, very pale eyes, and rapid breathing alongside a low Hb number means the body is struggling to carry enough oxygen.

Haemoglobin below 6 g/dL in most patients signals the need for urgent medical review and possibly transfusion.

3. HbF — Foetal Haemoglobin

🔬 What it measures: A special type of haemoglobin that does NOT sickle. Babies are born with high levels of it — which is why most sickle cell babies don't show symptoms at birth. As they grow older HbF naturally decreases and symptoms begin.

📊 Why it matters: Higher HbF levels = less sickling = fewer crises = milder disease overall.

✅ What a good result looks like: Higher is better. Patients with naturally high HbF often have milder sickle cell disease.

💊 What influences HbF: Hydroxyurea — a medication some patients are prescribed — works partly by increasing HbF levels. Consistent management and some natural compounds also support HbF.

This is one of the most important numbers in understanding how severe a patient's sickle cell is likely to be.

4. WBC — White Blood Cell Count

📊 Normal range: Roughly 4,000 to 11,000 cells per microliter — though sickle cell patients often run slightly higher naturally.

✅ What a good result looks like: Stable. Within or slightly above normal range.

🚨 A very LOW WBC can signal the immune system is suppressed — which makes the patient vulnerable to infections that a healthy person would fight off easily.

5. Platelet Count

🔬 What it measures: The tiny cells responsible for blood clotting. When you cut yourself and the bleeding eventually stops — platelets did that.

📊 Normal range: Roughly 150,000 to 400,000 per microliter. Sickle cell patients often have higher than normal platelet counts.

6. Reticulocyte Count

🚨 A SUDDEN DROP to near zero — called aplastic crisis — means the bone marrow has temporarily stopped producing new blood cells. This is serious and needs immediate hospital attention.

7. MCV — Mean Corpuscular Volume

🔬 What it measures: The average SIZE of red blood cells.

📊 Why it matters in sickle cell: Patients on hydroxyurea or with high HbF tend to have larger red blood cells — which is actually a positive sign that the medication is working.
