After examination the Emergency Doctor made a diagnosis of bilateral septal haematoma. All patients with nasal trauma should be assessed for septal hematoma, epistaxis, open fractures, and cerebrospinal fluid leak. A nasal septal hematoma arises from the collection of blood in the potential subperichondrial space. The hematoma inhibits adequate perfusion and can lead to cartilage damage in as little as 24 hours and necrosis within 72 hours. If not promptly treated, patients may develop future cosmetic distortion, including saddle nose deformity or secondary infection.

A 46-year-old man presented to the Emergency Department after falling from a standing position. His face struck the side of a concrete floor. He sustained facial lacerations and experienced obstructed nasal breathing as well as generalised midfacial pain. There was no evidence of cerebrospinal fluid leak. On examination, lacerations were present on the forehead and through the right nasal ala. Computed tomography of the facial bones demonstrated bilateral nasal bone fractures and ni intracranial haemorrhage.

A 43 year old man attended ED with shortness of breath and cough, and fever. A Chest X-ray was done, which shows an extensive bilateral upper-lobe predominant fibrocystic change, marked coarse reticular opacification, and multiple cystic/ring-shadow lucencies consistent with bronchiectatic and cavitary destruction, prredominantly affects both upper and mid zones, worse apically. There is also a volume loss in the upper lobes, and evidence of chronic scarring and architectural distortion.
